Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.� The solicitation is issued as Request for Proposal (RFP) 246-22-Q-0009. This procurement is full and open under NAICS code 561320 with a standard business size of $30 million.� The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06, effective April 26 2022.� The Indian Health Services intends to make a single award to a Native American/American Indian/Alaska Native owned� business, for a quantity of four (4) Medical Laboratory Scientist (MLS), also known as medical technologist (MT/MLT) or clinical laboratory scientist services, for the Lawton Indian Hospital located in Lawton, Oklahoma. � This combined synopsis/solicitation is 100% set aside as Buy Indian for a Native American/American Indian/Alaska Native owned vendor.� Proposals submitted by non- Native American/American Indian/Alaska Native owned vendors will not be accepted.� Any false or misleading information submitted by an enterprise when submitting an offer in consideration of an award set-aside under the authority of the Buy Indian Act is a violation of the law and punishable under 18 U.S.C. 1001. False claims submitted as part of contract performance are subject to the penalties enumerated in 31 U.S.C. 3729 to 3731 and 18 U.S.C. 287 Question/clarification submittal(s) can be emailed to Gabrielle.Gomez@ihs.gov.� No question/clarification submittal(s) will be taken after 08/24/2022 at 12:00 PM CST. All future information concerning this acquisition, including solicitation amendments, will be distributed solely through the Sam.Gov site. �Interested parties are responsible for monitoring this site to ensure you have the most up to date information about this solicitation.� No phone inquiries, please. � The closing date for receipt of proposals is September 8th, 2022.� Offers shall be submitted to the Oklahoma Area Indian Health Service (IHS), 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 12:00 p.m., on 09/08/2022.� The offer should be submitted in a sealed envelope, addressed to this office, showing the solicitation number, business name, point of contact name, and address.� Emailed quotes can be sent to the following information: Gabrielle Gomez, Contract Specialist, Gabrielle.Gomez@ihs.gov.� Vendors do not have the option to respond to select requirements and must be able to provide all requirements to be considered for this engagement. Furthermore, the vendor must have at least four (4) candidates available for assignment at the time of award.� The statement of work for this requirement is attached. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (attached, A.2)� CLAUSES INCORPORATED BY FULL TEXT: Please note that due to character limitation in SAM.GOV, full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulations (HHSAR) can be accessed on the Internet at http://www.farsite.hill.af.mil. In support to the information in FAR 52.212-1 & FAR 52.212-2 (attached, A.11) regarding evaluation details, prospective contractors shall provide the following: (1) Unique entity identifier from SAM.GOV (note: Contractors must be registered in the System for Award Management (SAM)) database prior to award, during performance and through final payment of any contract resulting from this solicitation.� Payments will be made using Electronic Funds Transfer (EFT). (2) All-inclusive firm fixed-price for Medical Laboratory Scientist (MLS), also known as medical technologist (MT/MLT) or clinical laboratory scientist services to include Option Year pricing for 2 years.� 12 months/Base Year for 1 MLS/MLT/MTs = $����������� /per hour���� quantity of 4 = $���� dollars 12 months/Option Year 1 for 1 MLS/MLT/MTs = $���������������� /per hour�� ��quantity of 4 = $����� dollars �� 12 months/ Option Year 2 for 1 MLS/MLT/MTs = $��������������� /per hour�� ��quantity of 4 = $����� dollars �� The Government will make one award to the most responsive and responsible vendor that offers IHS the �best value� price and technical factors considered. Since this �best value� approach contains price/technical tradeoffs, the award will not necessarily be made to the lowest price vendor or to the most technically qualified vendor. Note: Vendors that take exception to any term or condition of the RFP could make the submitted offer unacceptable to IHS. The vendor must show proof of current medical insurance in the appropriate amount of $1,000,000 per occurrence and $3,000,000 aggregate. The award may be made without discussions; therefore, vendors are encouraged to submit proposals that are responsive and most advantageous. If IHS deems discussions necessary, IHS will conduct discussion only with those vendors determined to be within the competitive range for an efficient award. The proposal submitted must contain at a minimum, two (2) relevant past performance references for the most recently performed contracts in which similar or the same services were performed by the offeror. References shall include the agency or customer name and address, contract number (if applicable), contract period of performance, contract description of services, type and quantity of personnel provided dollar value of contract, and point of contact information (name, phone, facsimile, Internet/e-mail address).� For each contract identified, the offeror shall also provide a discussion of problems encountered and their correction, as well as their accomplishments during the performance of the contract.� Offerors shall be evaluated on performance under the submitted relevant past performances.� Offers that have no record of past performance shall be assigned a neutral past performance rating and shall not be penalized if they do not have a past performance record. The Personnel Qualifications/Credentialing Packet to include current licensure & education for personnel providing Medical Laboratory Scientist (MLS), also known as medical technologist (MT/MLT) or clinical laboratory scientist services, can be from any US State or territory. The evaluation factors below are used by the technical evaluation committee when reviewing the technical proposals. The factors below are listed in the order of relative importance with weights assigned for evaluation purposes.� � � � 1. Personnel Qualifications - 40 Adequacy and feasibility of the proposed professional staff to work as Medical Laboratory Technician and, including Demonstrated ability to successfully manage blood sample processing and shipments to different laboratories and universities Demonstrated ability to implement the performance requirements described in this SOW Key personnel will have a minimum of qualifications and experiences as described in the Statement of Work. 2. Past Performance - 30 Experience in a laboratory setting or clinic Blood sample processing and shipment experience at research-based Universities or Institutions 3. Price Proposal Evaluation � 30 Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposal.� The Government is not responsible for locating or securing any information which in not identified in the proposal.� The Government reserves the right to make an award without discussions.
