Amendment 007: The purpose of Amendment 007 is to revise Section L 2.0 PASS/FAIL FACTORS as follows: - Delete: (iv) The Laboratory scope of accreditation shall include the ranges and uncertainties for the measurement areas required to perform the calibrations with the equipment identified in the total instrument inventory, provided as Attachment 1 to this solicitation. Please see attachment N6660422Q0136P0003. As a result the closing date and time of this solicitation is extended. All other terms and conditions of this solicitation remain unchanged. ___________________________________________________________________ Amendment 006: The purpose of Amendment 006 is to revise the Statement of Work�(SOW)�Section 4.0 as follows: - Delete:�The contractor shall provide valid ISO/IEC 17025 or ANSI Z540.3 Certificates of Laboratory Accreditation upon arrival to each facility. Every calibration shall be an accredited calibration. The accredited calibration shall include a Report of Calibration, before and after measurement data for each measurement, and the uncertainty associated with each measurement. �The certificate shall include a statement of compliance that states each calibration measurement meets or exceeds a 4:1 test uncertainty ratio (TUR), or ? 2% Probability of False Accept (PFA) and ? 15% Probability of False Reject (PFR).� - Insert:�The contractor shall provide valid ISO/IEC 17025 or ANSI Z540.3 Certificates of Laboratory Accreditation upon arrival to each facility. As a result the closing time and date of this solicitation is extended.� All other terms and conditions of this solicitation remain unchanged.� __________________________________________________________________ Amendment 005: The purpose of Amendment 005 is to provide responses to industry questions and extend the solicitation closing date. Please see attached ""Industry Q and A"" document. Solicitation closing date is extended to 6 July 2022 at 4 pm EST. All other terms and conditions of the solicitation remain unchanged. ________________________________________________________________________ Amendment 002: The�purpose of Amendment 002 is to�update language provided in Sections L and M. Please see attachment N6660422Q0136P0001. All other terms and conditions of the solicitation remain unchanged. ______________________________________________________________________________ Amendment 001: The purpose of Amendment 001 is to provide responses to industry questions. Please see attached 'Industry Q and A' document. All other terms and conditions of the solicitation remain unchanged. ________________________________________________________________________________ The Naval Undersea Warfare Center Division, Newport (NUWCDIVNPT) intends to award a Firm-Fixed-Price (FFP) Blanket Purchase Agreement (BPA) contract for calibration services in accordance with Solicitation N66604-22-Q-0136 (attached)� and the BPA information below. This requirement is being solicited on an unrestricted basis as concurred with by the NUWCDIVNPT Office of Small Business Programs (OSBP). The North American Industry Classification Code (NAICS) for this requirement is 811219. The small business size standard is $22 million. BLANKET PURCHASE AGREEMENT INFORMATION (REQUIRED PER FAR 13.303-3 �PREPERATION OF BPAs�) (1) Description of Agreement. The supplier shall provide calibration repair service as described in the Statement of Work (SOW) located in Section C, if and when requested by the individuals identified in section 4 below. (2) Extent of obligation. The Government is obligated only to the extent of authorized purchases actually made under the BPA. (3) Purchase limitation. The dollar limitation for each individual purchase under the BPA is the Simplified Acquisition Threshold. (4) Individuals authorized to purchase under the BPA. The individuals authorized to purchase under the BPA are as follows: a. For all Orders of any value a warranted Contracting Officer at the Naval Undersea Warfare Center Division, Newport within the limitation of each individuals specified warrant is authorized to purchase under this BPA. b. This BPA permits only the Government employees specifically authorized, to place calls against it. THIS MUST BE STRICTLY ADHERED TO. c. Any services rendered pursuant to a call by an unauthorized person are performed at the contractor�s risk and may result in non-payment and possible cancellation of this agreement. (5) Delivery tickets. All submissions under the agreement shall contain the following minimum information: a. Name of Supplier b. BPA number c. Date of order call d. Order call number e. Itemized list of services furnished f. Unit price of the services g. Date of completion (6) Invoices. Invoices shall be submitted for each individual Calibration and Repair service provided to the Government. No third party billing is allowed (i.e., PaypalTM) a. For all purchases, the invoice shall be submitted in accordance with the appropriate clause in Section G.
