Description
Laundry Dryer Lint Cleaning Service Department of Veterans Affairs (VA) Veterans Health Administration (VHA) BRIEF SCOPE OF WORK (This is for information only) PURPOSE: Veterans Affairs requires a contractor to provide quarterly routine servicing of two indoor laundry dryer vents at the Oklahoma City VA Health Care System. Services are to be performed in accordance with this PWS, OEM standards and NFPA Fire Code, with the intent of ensuring the equipment operates as the original manufacturer intended, in a cost-effective, efficient manner, contributing to the VA Mission. BACKGROUND: Equipment: The following systems of equipment require service under this contract. EQUIPMENT DESCRIPTION BRAND NAME MODEL Laundry Dryer Energenics J8 /J80400 Screen TASKS AND REQUIREMENTS: The Contractor shall provide all labor, materials, and equipment necessary to remove and properly dispose of all lint, dust, and debris from dryer vents in accordance with all standard commercial, state and local practices and with all current and applicable Environmental Protection Agency (EPA) and Occupational Safety and Health (OSHA) requirements. The contractor must furnish Engineering Service a detailed field report, or equivalent, showing work has been completed before leaving the Facility. Laundry dryer vents shall be considered clean when waste materials have been fully removed. Contractor shall be responsible for clean up around the dryer(s) area as necessary from the act of removal. The operator shall verify completion of work, sign service ticket, and retain a copy for VA records. Contractor shall not replace any parts or perform any service that will result in additional charges without prior approval from the VA Contracting Officer Representative (COR) at the specific site. The COR will be determined at a later date. Water and electricity are available at each dryer site and may be utilized by the contractor at no cost in quantities necessary to perform the work herein. The contractor shall be responsible for all temporary connections necessary for operations and for removal prior to final acceptance of work. LIMITATIONS OF SUBCONTRACTING: The Limitations of Subcontracting may or may not apply to a resulting solicitation from this Market Research. Vendors are advised to familiarize themselves with Federal Acquisition Regulation parts 19.505(a)(2) and 19.505(b)(1) as it pertains to subcontracting limitations. Vendors are also advised to familiarize themselves with 13 CFR 125.6 and the penalties described in this regulation. As Primary Contractor: what % of Total Contract Value will you retain? b. If using a Subcontractor: what % of Total Contract Value will go to them? 3. Please provide SAM Unique Entity ID number. 4. If you have an FSS/GSA contract that includes this service. 5. Provide FSS/GSA contract number if applicable.
