- 2 Statement of Work WATER MAIN REBUILD LOOP ISOLATION VALVES, ADD HYDRANT SERVICE VALVE, REPLACE EXISTING HARDWARE Stratton VAMC Albany Introduction- Stratton VAMC s mission is to provide world class healthcare for veterans in the capital region and upstate New York. Background /Objective The Stratton VAMC, is currently seeking a vendor to provide all labor, materials, equipment and travel to rebuild two (2) 12 Clow loop isolation valves, add one (1) hydrant service valve and replace all hardware on existing valves and flanges in the designated area (see Att. A). During a similar repair in March of this year the housing, couplings and valves were found to have unusual wear and deterioration which caused them to leak. The presumption is that other buried fittings may be in the same or worse condition. This requirement will give the Medical Center more information on the condition of and rate of deterioration of the water main, valves and fittings in order to make an informed decision on a course of action to prevent any more failures to our water mains and fire suppression systems.. Place of Performance Stratton VAMC Albany NY, 113 Holland Ave Albany, NY 12208 Period of Performance The services POP 1 week after all materials have been gathered and not to exceed 1 week of work. Scope of Work Provide all labor, materials, equipment and travel to excavate rebuild/repair/install valves and replace hardware in one location of main water system. Specific Tasks Contractor shall furnish all parts and labor for full traffic control of work areas. Contractor will excavate around the designated valve assemblies, which are approx. 39 from grade to the top of the square drive on the valves. Contractor will rebuild two (2) Clow 12 resilient wedge loop isolation valves. This rebuild will include new brass stem, and replacement of the wedge, along with necessary bonnet gaskets, or O rings. Parts are to be the equivalent make and model as the existing item replaced. Contractor will install a new hydrant service valve on the hydrant #3 branch line. Install will include a 6 -8 Clow resilient wedge valve, along with a valve box and cover installed to match the elevation of the existing loop valves. Contractor will replace all existing T bolts, nuts, and MJ glands with bolts and heavy hex nuts that are to be 304 or 316 stainless steel, manufactured in accordance with ANSI/AWWA C111/A21.11. MJ glands are to be replaced with new glands that meet ANSI/AWWA C-110/A21.10 and ANSI/AWWA C111/A21.11 Standard. Contractor will install #2 stone bedding to stabilize the bottom of the excavated trench and to give a solid platform for valve assemblies. Contractor will backfill with new, dry run-of-bank gravel material compacted in 12 lifts. and top with 6 of screened soil Contractor will Set/Reset all telescopic valve boxes atop water valves with box top even with finish grade. Additional Information - The water main at this location is 12 SDR9 HDPE plastic, with MJ flange adaptors and there are (2) loop isolation valves inline (valves 8 & 9 on attachment A), along with a tee between the valves that serves hydrant 03. The VA has field verified the operation of the existing valving, and it is operational, and also field verified that we can isolate the area, to allow the hardware to be replaced. Historically on the property the VA has found the hydrant branches to be 6 diameter piping, but have found a couple of 8 lines as well Non-compliance - The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default. Government Furnished Property- N/A. Inspection - Upon installation of new parts the COR/POC will perform the inspection to ensure that the system is returned to full functioning condition No invoices will be processed for payment prior to the completion of POC inspection. Other Related Services (Not Included in the Scope of Work) Any services not expressly stated in this statement of work are not authorized. Services such as inspection, Original Equipment Manufacturer (OEM) required maintenance, or other maintenance and repair vital to the continued functioning of the systems may be added only after written modification from the contracting officer, unless they are inclusive to the requested services. No other person, including the COR or any other VA personnel are authorized to request additional work be performed by the contractor. Any additional services performed that are not authorized by the contracting officer will be considered inclusive. Attachment A Attachment B
