- Statement of Work Kitchen Hood Cleaning Service Background: Oklahoma City VA Medical Center (OKCVAMC) has a requirement for routine servicing per NFPA of three indoor kitchen vent hoods which are located at Nutrition and Food Service (1) the Canteen Service (2). Hoods will be maintained quarterly, as required by NFPA Fire code. Statement of work: The Contractor shall provide all labor, materials and equipment necessary to remove and properly dispose of non-hazardous oil, grease, and sludge from kitchen hoods located at the OKCVAMC. The Contractor shall transport and dispose of all waste in accordance with all standard commercial, state, and local practices and with all current and applicable Environmental Protection Agency (EPA) and Occupational Safety and Health (OSHA) requirements. Services include removal of waste and grease build-up from hood walls and all associated equipment within the hoods. The entire hood area shall be cleaned with water and any remaining residue pumped out. The Government will furnish water necessary to complete the maintenance. The Contractor shall file any paperwork required with the City Fire Marshal to prove the service was performed and meets standards. Performance: All contractor personnel shall report to the Maintenance Supervisor in Engineering Office at 921 N.E. 13th St Oklahoma City, OK to sign in. The contractor must furnish Engineering Service a detailed field report, or equivalent, showing work has been completed before leaving the Facility. The contractor shall not discharge chemicals or grease into the OKCVAMC sewage system, or any municipal or regional sewerage works, public or private. Kitchen hoods shall be considered clean when waste materials have been fully removed. Contractor shall be responsible for clean up around the hood(s) area as necessary from the act of removal. The operator shall verify completion of work, sign service ticket, and retain a copy for VA records. Contractor shall not replace any parts or perform any service that will result in additional charges without prior approval from the VA Contracting Officer Representative (COR) at the specific site. The COR will be determined at a later date. Water and electricity are available at each hood site and may be utilized by the contractor at no cost in quantities necessary to perform the work herein. The contractor shall be responsible for all temporary connections necessary for operations and for removal prior to final acceptance of work. The contractor is responsible for his employees while on Facility grounds including any accidents while performing the service. Contractor is responsible for ensuring that his employees are wearing appropriate VA Contractor Badges in a visible location (above the waist) while performing contracted duties at the OKCVAMC. Contractor shall immediately notify the COR of any deficiencies found during any phase of the kitchen hood cleaning. Qualifications: The Contractor personnel shall possess necessary skills, experience and education required to clean industrial kitchen hoods. All maintenance services must meet applicable safety standards. Work Hours: The following terms have the following meanings and are applicable to this contract: Normal hours of coverage are Monday through Friday from 8:00 am to 4:30 pm, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR or his designee. Work performed outside the normal hours of coverage must be approved by Chief of Engineering and the COR. Funding for services that are outside the normal coverage hours or on holidays must have funding approved prior to any service being allowed.
