Description
Department of Veterans Affairs Veterans  Health Administration (VHA) Sources Sought Notice   Obtain a qualified vendor to Replace boom medical gas outlets and inlets. This is a Sources Sought notice and not a request for quotes. This request is solely for the purpose of conducting market research to enhance VHA s understanding of your company s offered services and capabilities. The Government will not pay any costs for responses submitted in response to this Source Sought Notice. This Sources Sought notice provides an opportunity for respondents to submit their capability and availability to provide the requirement described below. Vendors are being invited to submit information relative to their potential to fulfill this requirement, in the form of a capability response that addresses the specific requirement identified in this Sources Sought, please ensure that you indicate your DUNS number on your response. The Veterans Health Administration (VHA) is seeking to obtain a qualified vendor Obtain a qualified vendor to Replace boom medical gas outlets and inlets at the West Haven VAMC per the general requirements below. The standard shall be of quality; meeting or exceeding those outlined in the general statement of work as described below. Refer to the statement of work section below for the requested requirement description. This Sources Sought is to facilitate the Contracting Officer s review of the market base, for acquisition planning, size determination, and procurement strategy. STATEMENT OF WORK OR Suite, Replace Boom Medical Gas Outlet/Inlet GENERAL INFORMATION 1. Title of Project: OR Suite, Replace Boom Medical Gas Outlet/Inlet. 2. Project Location: VA Connecticut Healthcare System, 950 Campbell Avenue, West Haven, CT 06516 3. Background: The parts and pieces in the assemblies, in most cases, are original equipment and age is now becoming a major factor due to maintenance and repair required annually. 4. Scope of Work: The Contractor shall provide all labor, materials, tools and equipment to complete below work and provide complete testing and certification. a) Replace all medical pressure gas outlets and medical vacuum inlets as positioned on TASC 2000 articulated boom pendants located in 3rd floor OR s 1 7. Replace on combined two (2) articulating boom pendants in OR s 1, 3, 4, 5, 6 & 7: 1. (4) Oxygen outlets 2. (6) Medical vacuum inlets 3. (2) Each medical air, nitrous oxide (n2o), nitrogen (n2) outlets Replace on one (1) articulating boom pendant in OR 2: 1. (2) Oxygen outlets 2. (3) Medical vacuum inlets 3. (1) Each medical air, nitrous oxide (n2o), nitrogen (n2) outlet b) Per NFPA 99 health care facilities code [5.1.14.2.3] inspection and testing operations and [5.1.14.2.3.1] manufactured assemblies employing flexible connection(s) between user terminal and the piping system. Maintaining system operating pressure all connections and joints shall be examined for leakage. 2. Safe working condition of the flexible assemblies shall be confirmed. 3. D.I.S.S. connectors internal to the boom and articulating assemblies shall be checked for leakage. 4. Leaks, if any, shall be repaired (if permitted) or components replaced (if required). The project shall be accomplished in compliance with all applicable standards and codes specified in the VA Design Guides and Manuals. VA specifications can be found at the VA Technical Information Library: Technical Information Library (TIL) - Office of Construction & Facilities Management (va.gov) 5. Performance Period: The Period of Performance is 60 calendar days from Notice to Proceed date unless otherwise directed by the Contracting Officer (CO) in writing. 6. Normal Work Hours: Work can only be performed with advance notice Monday thru Friday, 7:00pm thru 6:00am each day, or Saturdays and Sundays. 7. National Holidays: The 11 holidays observed by the Federal Government are: New Year s Day, Martin Luther King s Birthday, President s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. B. CONTRACT AWARD MEETING The contractor must not begin work until the CO has issued a Notice to Proceed (NTP). A contract kick-off meeting will be held prior to issuance of NTP. Material submittals shall be submitted to the COR within 10 calendar days of the NTP. C. DELIVERABLES The contractor shall submit electronically each material for approval prior to installation. Materials shall not be ordered or installed prior to approval of the submittals. Direct all submittals to the designated COR through the CO. Certification from Contractor/Installer that the system specified meets all identified codes as required by the Government. REFERENCES BUT NOT LIMITED TO: Division 01 General Division 22 Plumbing dmPlbg - PLUMBING DESIGN MANUAL 22 05 11 - Common Work Results for Plumbing 22 63 00 - Gas Systems for Laboratory and Healthcare Facilities ATTACHMENTS: Material Approval Submittal Plumbing Design Manual 22 05 11 - Common Work Results for Plumbing 22 63 00 - Gas Systems for Laboratory and Healthcare Facilities *The contract further requires the vendor to plan for, establish and implement procedures and scheduling practices to minimize, to the greatest extent possible, the impact to the work performed at the medical center. GENERAL SPECIFICATIONS Standards: Contractor shall meet all Federal/ VA, state, and local standards, codes, and requirements. All design and installation shall be per master specifications and design manuals, and design guides found on the VA Technical Information Library. Parking: Parking is rigidly controlled throughout the Medical Center. Parking of vehicles in other than the specified areas will result in said vehicles being towed at the owner s expense off the Medical Center grounds. Staging Areas: Onsite staging areas, including storage and dumpster locations, will not be provided, or allowed for this contract. Safety: The contractor shall be responsible for developing their own safety plan for all site work, including asbestos awareness training and procedures when encountering hazardous materials (i.e. asbestos, lead, etc.). Contractor shall notify VA of any concerns to suspect hazardous or regulated materials. Plan shall also include contractor s plan and measures to keep contractor employees, and VA staff and patients safe in relation to COVID 19. Contractors working onsite at the VA must wear face coverings at all times provided by the contractor. For external building work, contractors shall screen their own employees daily meeting Center for Disease Control and Prevention guidelines. Contractors working internal to the buildings on VA property shall go through VA screening each day. This plan must be approved by the VA. Scheduling of Work: Upon receipt of the contract award, the Contractor must contact the CO and his/her COR to schedule the work. Work Completion: All work under this contract shall be completed within 60 days following the award of the contract and the issuance by the VA of the Notice to Proceed letter. Completion of the contract includes inspection of all installed project features by the VA COR or designated alternate representative. Material & Workmanship Quality: All materials and workmanship shall comply with all applicable codes and standards. All work shall be done in a first-class manner according to the best trade practices and to the satisfaction of the COR. DETAILED DESCRIPTION: All work performed under this contract shall be in accordance with the VA Master Specifications. The VA Master Specifications may be found at http://www.cfm.va.gov/til/. Quality Control Plan: Vendor shall establish a quality control plan for all work, including that of subcontractors, to assure compliance with the contract provisions. Quality control measures shall include, but not be limited to: Obtaining payment and performance bonds, permits, licenses and insurance Listing all subcontractors and services they will perform Submittal approval Inspection schedule Compliance with applicable federal, state, and local codes and regulations related to executing the contract Correction of unsatisfactory work Safety: Contractor shall comply with the following and always keep a copy of the attached-on site: Contractor Safety Pamphlet CT - VAMC Security: All contractor personnel shall obtain a short-term identification badge issued by the VA Connecticut Healthcare System. Coordinate through the Project Manager/COR. Such badge shall be worn by the individual and prominently displayed at all times while on VA property. No employee of the contractor shall enter the project site without a valid identification badge issued by the VA. In order to obtain a short-term identification badge, contractor personnel shall present to the Project Manager (or designee) a valid (non-expired) photo identification issued by a US federal, state or local government agency. All contractor personnel are subject to inspection of personal affects when entering or leaving the project site. Records Management: Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. End of SOW
