Change 0002 Updated RFQ number from W912CN-22-Q-0039 to W912CN-22-0048 under (xvi) Government RFQ Point of Contact and uploaded updated CSS in the attachment section to reflect changes.�� Combined Synopsis/Solicitation i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This enclosure is an addendum to FAR provision 52.212-1, Instructions to Offerors � Commercial Items, which applies to this acquisition. (ii) Competitive quotes are being requested under Request for Quote (RFQ) W912CN-22-Q-0048. (iii) This solicitation document incorporates provisions and clauses are those in effect through Federal Acquisition Circular FAC 2022-06, Effective: Effective May 26, 2022. (iv) This procurement is unrestricted and will be full and open for competition in the marketplace. (v) DUE DATE AND SUBMISSION INFORMATION Formatting Requirements: Submit quotes in electronic PDF. Period of Performance: 01 September 2022 - 31 August 2027. � The place(s) of the performance is: Bassett Army Community Hospital (BACH) at Fort Wainwright in Fairbanks, Alaska 99703. (x) OFFEROR INSTRUCTIONS The provision at 52.212-1, Instructions to Offerors � Commercial Items (NOV 2021), applies to this acquisition.� The Government intends to award a Firm-Fixed-Price contract resulting from this solicitation to the responsible offeror whose quotation conforming to the solicitation is most advantageous to the Government as defined in the �EVALUATION FACTORS FOR AWARD� section below.� A complete quote must be received for consideration.� The Government may consider quotes that fail to address or follow all instructions to the non-responsive and ineligible for contract award.� �A complete quote includes a response and submission to each of the following: General Information: Offeror Business Name, Address, and CAGE code.� Unique Entity Identifier (ensure Representation and Certifications are up to date in SAM.gov), Business Size and Type of Small Business based on applicable NAICS Code as provided above, Federal Tax ID, Primary Point of Contact (to include telephone and e-mail address).� Reference FAR provision 52.212-3, Alternate I, Offeror Representations and Certifications � Commercial Items, applies to this acquisition.� Offerors must include a completed copy of this provision with offer. Price Quote: Submit complete pricing for each individual item listed in the CLIN(s) above to include the unit of issues, the extended price for each line item and the total price in US Dollars ($). System for award Management (SAM) registration: The vendor must be actively registered at the time of offer submission.� Registration can be completed at https://www.sam.gov. Procurement Integrated Enterprise Environment (PIEE) and Wide Are Workflow (WAWF) Registration:� Contractors MUST also be actively registered in PIEE with the appropriate roles in WAWF in order to get invoices be paid.� Paper copies will not be accepted.� Registration can be completed at https://piee.eb.mil. (xi) Evaluation Factors for Award: In lieu of the provision at 52.212-2 Evaluation-Commercial Products and Commercial Services, the specific evaluation criteria included are CERTIFICATION and PRICE defined below: Factor I - Certification: Offeror must be a factory-trained technician as part of the Ascom Certified System Integration program in order to install, service and access manufacturer tech support.� Offeror must include a certificate of training or verification from Ascom via email or other documentation. Factor II � Price:� The Government will evaluate the total price to determine if it is fair and reasonable IAW FAR 13.106-3. The price quote shall include a unit price for each item and a total firm-fixed-price for all line items. The total firm-fixed-price shall include all applicable taxes and all associated price as outlined in paragraph (v) above. **Basis for Award: The Government intends to award a firm-fixed-price contract to the responsible offeror meeting solicitation requirements, such as terms and conditions, representations, and certifications in order to be eligible for award. The award decision will be based on price and other factors IAW FAR 13.� The Government will assess Factors I and II and determine which is the best value to the Government. Quotes will be evaluated for price fair and reasonableness. Offerors are cautioned that an unrealistically low price, as proposed initially or subsequently, may be a ground for eliminating a quote from consideration, either on the basis that the offeror does not understand the required services, or he/she has made an imprudent offer.
