- STATEMENT OF WORK AUGUST 12, 2022 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for healthcare furniture for San Francisco VA Clinic located at 4150 Clement St, San Francisco, CA 94121, that meet high-quality threshold while integrating excellent environmental stewardship. This requirement includes healthcare furniture, design, project management, professional installation and maintenance/warranty services. The Contracting Officer Representative (COR) is Christina Eddy. Healthcare furnishings include, demountable walls, acoustic wall tiles, and related services. The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and be responsible for the technical assistance, development and generation for final specifications. Furniture delivery 4150 Clement St, San Francisco, CA 94121. Bldgs 22 & 203. Furniture and Installation services are needed for August September 2022, or earliest possible date thereafter. Duty hours will be 8am 4:30pm M-F. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Juneteenth National Independence Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide project management of all products and services to manage, order, ship, deliver and install new furniture from the manufacturer until final acceptance by San Francisco VA Health Care System Prepare and provide furniture drawings in PDF and AutoCAD Drawings to include the following: Installation drawings for installers to use during install may include panel diagram and electrical plans. VA drawings may not be enough for the installers. The VA will not be creating additional drawings for the installers. 3D drawings of these areas: Track furniture through ordering, delivery schedules, shipping, and installation. Provide schedules and tracking to VA Interior Designer. Provide local Project Manager to be physically onsite during deliveries, and installation and manage installation. Print out drawings needed for the installers. VA will not print out drawings for the installers. Before installing all wall mounted items coordinate and get approval from Interior Designer. Complete a final walk with lead installer(s) and compile a punch-list . Manage punch list items, including the ordering, delivery, and install of punch list items. Provide maintenance manuals for all products specified, if necessary. 3.0 KICKOFF MEETING The prime contractor(s) shall participate in a kickoff meeting within ten (10) days of task order issuance, using Microsoft Teams meeting. Time and date will be determined by VA Point of Contact. 4.0 SUBMISSION OF QUESTIONS Questions: All questions must be received by July 12, 2022. All questions submitted for this solicitation must be electronically sent to the following email address: Felicia A. DeMita at felicia.demita@va.gov. Questions received after the stated date and time, may be addressed at the discretion of the Contracting Officer. 5.0 SUBMISSION OF QUOTATION See FAR 52.212-1 Instructions to Offerors Commercial Items - Addendum 6.0 EVALUATION CRITERIA see solicitation at FAR 52.212-2 Evaluation Commercial Items 7.0 PRODUCT General All finished surfaces shall be free of scratches, mars, dents, or blemishes, and withstand staining, and exhibit to flaking, cracking, or loss or adhesion. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. Product may utilize Quick ship option when available and is preferred. Locking requirements lever handle. The ceiling height is approximately 9 , but dimensions will need to be verified on site before the order is placed. The areas for the demountable walls are noted on the drawings. 8.0 SERVICES Design Contract Tasks/Requirements The contractor shall attend a minimum of two (2) meetings on Microsoft Teams with the VA Interior Designer to review final award. During the first meeting the contractor shall review the scope and ask any questions. During second meeting the contractor shall provide the Interior Designer with the Auto-Cad drawings to [1/4 1/8 ] scale drawings and 3D drawings showing layouts of awarded product in noted areas from section 2.C.b The contractor shall allow for two (2) revisions per department layout included in design services including updating Auto-Cad Drawings and PDFs as request by VA Interior Designer. The contractor shall be responsible for taking and applying accurate field measurements to ordered product for verification of correct sizing. Auto-Cad drawings may not be accurate finished dimensions and may not be used in place of field measurements. The Vendor shall be liable for any incorrect field measurements leading to incorrect product order. The contractor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA Interior Designer. The product must not be placed into production without clearance from the VA Interior Designer. The contractor shall track manufacturing schedule, shipping, and receiving schedules and notify VA Interior Designer with updates of estimated date by email. The contractor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to the VA Interior Designer a remediation plan within 5 business days. The contractor shall provide up to 30 days free storage if any small construction delays. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. Project Management Track furniture through ordering, delivery schedules, shipping, and all the way through installation. Provide schedules to ID COR and Project Manager. Provide a local Project Manager to be physically onsite during deliveries, Installation and managing installation. Print drawings requested by installers. Before installers install all wall mounted items coordinate with Interior Design for approval before install all other wall mounted items (ie clocks, whiteboards, etc.) Complete a final walk with lead installer(s) and compile a punch-list . Manage punch list items, including the ordering, delivery, and install of punch list items. Provide maintenance manuals for all products specified if necessary. Work Required by Others All work required by other trades shall be identified under this section by the vendor. Questions/Answers 0001 - Bldg 22, Corridor.
