Page 4 of 4 STATEMENT OF WORK Building 76 4th Floor North Corridor and HopTel Flooring Replacement VA Finger Lakes Healthcare System, Bath Facility 76 Veteran s Ave., Bath, NY 14810 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for healthcare furniture for VA Finger Lakes Healthcare System, Bath Facility for Building 76 4th Floor North Corridor and HopTel Flooring Replacement that meet the VHA specifications and high-quality threshold while integrating excellent environmental stewardship. This requirement includes VA approved flooring material ordering, delivery, flooring preparation and professional flooring and base cove installation. Flooring project includes removing mastic from existing terrazzo cove molding, removing mastic/prepping flooring area per manufacturing instructions and installing LVT in corridors and flocked flooring in HopTel rooms, and then install base cove to finish. Note that existing carpeting removal shall be done by VA in-house shops prior to the contract install scheduled time. **The square footage of the corridor/LVT would be 1,100 sq ft. The square footage of the rooms/Forbo Flotex would be 1,350 sq ft.** The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. 2.0 PERFORMANCE REQUIREMENTS Project delivery and installation in Building 76 4th floor North is contingent on the existing engineering project Building 76 4th Floor Ventilation Contract to be complete in 4th Floor North for this contract to begin install. Duty hours will be M-TH 7a-5:30p. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide project management of all products and services to manage, order, ship, deliver and install new flooring from the manufacturer, per manufacturers specifications until final acceptance by POC Interior Designer. Contractor is required to have on site staff supervisor with OSHA 10 certification. This project can be done during normal working hours as there is no one moved into the space at this time. Provide maintenance and warranty information on products specified and professional installation. Minimum Technical Requirements: Category: Flooring   GENERAL REQUIREMENTS FOR Carpet Flooring: Basis of design: Forbo Flotex Lava II plank carpet tile Slip Resistance: Meets or exceeds the industry recommendation of >0.5 for flat surfaces when tested in accordance with ASTM D 2047, Standard Test Method for Static Coefficient of Friction. Suitable for use on flat or inclined surfaces. Acoustic Properties A.: Delta Lw=17db when tested in accordance with ISO 140-8 Measurement of sound insulation in buildings and of building elements. Acoustic Properties B.: 0,10 when tested in accordance with SOP 354, Measurement of Sound Absorption in a Reverberation Room Acoustic Properties C.: IIC 59 when tested in accordance with ASTM E 492, Standard Test Method for Laboratory Measurement of Impact Sound Transmission Through Floor/Ceiling Assemblies using the Tapping Machine. waterproof General description: 100% nylon type wear layout with intermediate fiberglass layer and recycled vinyl cushioned backing. Fire Testing: Class 1 when tested in accordance with SSTM E 648/NFPA 253, Standard test Method for Critical Radiant Flux. Wear Layer Density: over 70,000,000 nylon 6.6. fibers per square meter. Plank size/Gauge: 9.8"" x 39.37"" (+/- 1"") and .19"" gauge. Environmental A.: Compliant with CDPH 01350 requirements for VOC emissions and indoor air quality. Environmental B.: Contributes to LEED v4 credits Durability A.: Class 4 (heavy duty) when tested in accordance to EN 1307, Classification of Pile Carpets (Wear Classification). Very slight color change when tested in accordance with AATCC 165 (Colorfastness to Crocking Wet & Dry). Floor Care: Cleanable with water, a floor scrubber and a neutral pH cleaner such as Diversey brand cleaning products used by our EVS staff. Warranty: Limited 20-year warranty.   GENERAL REQUIREMENTS FOR LVT Flooring: Basis of Design: 6 Degrees Compass wood-look plank LVT Slip Resistance: Meets or exceeds the industry recommendation of >0.5 for flat surfaces when tested in accordance with ASTM D 2047, Standard Test Method for Static Coefficient of Friction. Suitable for use on flat or inclined surfaces. Wear Layer: 28 mil wear layer enhanced with the addition of UV cured, ceramic-reinforced urethane finish. Infused with EnviroSD Anti-bacterial Agent Suitable Installation Locations: Heavy commercial traffic, hospital, retail. Acclimation time: 48 hours at 65 - 85 degrees F. Environmental A.: locally sources raw materials - made in the USA. Environmental B.: 100% recyclable Environmental C.: Qualifies for LEED v4/4.1 credits Environmental D.: GreenGuard Gold Certified, meets CA 01350 requirements. Plank size/gauge: 6"" x 48"" x 3mm Resistances: Excellent resistance to chemicals, indentation, dimensional instability, heat instability, light instability. Floor Care: Cleanable with water, a floor scrubber and a neutral pH cleaner such as Diversey brand cleaning products used by our EVS staff. Requires no top coat or sealer, wax or other finish treatments. Warranty: 20-year commercial warranty.   GENERAL REQUIREMENTS FOR Rubber Base Moulding: Basis of Design: Roppe 6"" Cove Base Lengths: 100' coil About 600 linear ft **GENERAL REQUIREMENTS FOR Thresholds** Each doorway in drawing will require a threshold (about 96 linear ft) Flat Schlueter strip to edge the LVT 3.0 PRODUCT General All finished surfaces on flooring and cove molding shall be clean and free of scratches, mars, dents, or blemishes, and exhibit no flaking, cracking, or loss of adhesion. Flooring shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing or furniture. Items are desired to match existing facility standards which are: 6 Degrees Compass wood plank LVT in corridors, color/pattern TBD Forbo Flotex Lava II plank carpet tile in HopTel rooms, color/pattern TBD Roppe 6 Cove Base Coil, color TBD 4.0 SERVICES Design Contract Tasks/Requirements The contractor shall attend a minimum of one (1) in-person meetings at the Building 76 4th Floor North to review final award with VA Interior Designer and End Users and to make necessary revisions. The contractor shall be responsible for taking and applying accurate field measurements to ordered product for verification of correct sizing. Auto-Cad drawings may not be accurate finished dimensions and may not be used in place of field measurements. The Vendor shall be liable for any incorrect field measurements leading to incorrect product order. The contractor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA Interior Designer. The product shall not be placed into production without written clearance from the VA COR. The contractor shall provide final, clean [1/4 1/8 ] scaled drawings of product layout for sign-off prior to scheduling manufacturing. The contractor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email. The contractor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to the VA Interior Designer a remediation plan within 5 business days. The contractor shall provide 30 days free storage. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. 5.0 KICKOFF MEETING The prime contractor(s) shall participate in a kickoff meeting within ten (10) days of task order issuance, in person at B76 4th floor north. Time and date will be determined by VA Point of Contact.
