This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.� � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � �� Solicitation # RFQ-22-PHX-049 and this notice is issued as a Request for Quotation (RFQ).� This is a combined synopsis/solicitation for commercial commodities/services. Submit written quotes only, oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM). This requirement is under North American Industrial Classification Standard (NAICS) codes: 541519; or any applicable NAICS code, along with the appropriate Small Business Size Standard.� This requirement is a 100% set-aside for Small Business. A tiered evaluation preference will be used to first evaluate quotes from Indian Small Business Economic Enterprises (ISBEE); if no award can be made to an ISBEE, quotes from other small business (SB) socio-economic designations will be reviewed; if no award can be made to a SB, other than small business quotes will be reviewed. �� ISBEE vendors must complete the attached�INDIAN HEALTH SERVICE BUY INDIAN ACT INDIAN ECONOMIC ENTERPRISE REPRESENTATION FORM.� REQUIREMENTS:� Indian Health Service (IHS) � IHS Phoenix Area office provides IT services to eleven regional areas referred to as service units. Service units are located across central Arizona, northern Nevada and Northwestern Utah. The facilities range from small ambulatory care clinics to full service hospitals. Most of the facilities are located on Indian reservations. The existing 15 locations are currently running Cisco Unified Communication (UC) systems of various age and configuration. The central system will include Cisco Unified Communications Manager (CUCM), Cisco Unity Connection (CUC), and Singlewire InformaCast for Paging. This procurement is for NEW Equipment ONLY; no remanufactured or ""gray market"" items. Vendor shall be an Original Equipment Manufacturer (OEM authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All, warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions. All Equipment must be covered by the manufacturer's warranty. QUOTES MUST INCLUDE A COPY OF THE AUTHORIZED DISTRIBUTOR LETTER OR OTHER FORM OF EQUIPMENT VERIFICATION FROM THE MANUFACTURER TO VERIFY THE SOURCE OF THE PRODUCTS BEING QUOTED. Failure to provide material proof that the items requested are coming from an authorized channel from the OEM, may deem a quote non-responsive, and may not be further reviewed. This is a mechanism for the Government to verify OEM direct equipment, OEM warranty status, new condition (versus refurbished) and other applicable, implied OEM terms and conditions, such as FDA device approvals, etc. REQUIRED ITEMS: Items in accordance with the attached Statement of Work (SOW), and Bill of Materials (BOM). TECHNICAL PROPOSAL: Offerors are required to submit both a written technical proposal and a price quote to Government officials for the purposes of assuring that the prospective Contractor is fully cognizant of the scope of this contract and has the capability to complete all Statement of Work (SOW) requirements. Offerors shall provide a technical quote that includes the following three general areas: Management Approach, Technical Capabilities, and Installation Schedule; Personnel Qualifications; Organizational Experience. PAST PERFORMANCE: The Offeror shall identify a maximum of five (5) contracts/task orders with the Federal Government and/or commercial customers that demonstrate recent and relevant past performance.� Recent is defined as within the last three years from solicitation close date. Relevant is defined as any contract or subcontract valued at $250,000 or greater with a similar type requirement. Relevant Past Performance evaluation via Past Performance Information Retrieval System (PPIRS). PPIRS records for the identified contract will be assessed and other records within PPIRS may be considered. No submission is required for this section. PRICE QUOTE: The price quote is to be submitted as a Firm Fixed Price quote using the equipment list as a Price Schedule.� The attached Price and Cost Schedule should be filled out completely with the total price of the contract for every estimated year and total. Price will be evaluated in whole, including all service options and optional accessories quoted. Failure to provide quotes for items may be deemed non-responsive and may not be further evaluated. The Government will select the quote that represents the best benefit to the Government at a price that can be determined reasonable. The Government will perform a comparative evaluation of technical solution, past performance, and price. The major evaluation factors for this solicitation include technical (which encompasses management approach, technical capabilities, installation schedule, personnel qualification, and organizational experience), past performance and price factors.� Technical and Past Performance when combined are more important than price.� In any case, the Government reserves the right to make award to that offeror whose quote provides the best overall value to the Government.� Offerors are advised that award will be made to that offeror whose quote provides the combination of features that offers the best or greatest overall value to the Government.� The evaluation will be based on the demonstrated capabilities of the prospective Contractors in relation to the needs of the project, as set forth in this combined synopsis.
