- NOTICE OF INTENT TO SOLE SOURCE REQUIREMENT The Department of Veterans Affairs, LTC Charles S. Kettles Ann Arbor VA Medical Center, Network Contracting Office (NCO) 10, Pathology and Laboratory Testing (113), 2215 Fuller Road, Ann Arbor, Michigan 48105-2303. Intends to Negotiate a Sole Source Contract that will include a Base Year, Plus Four (4) Option Year Renewals under the Simplified Acquisition Procedures and the authority of FAR Part 13.106-1(b), Only One Responsible Source for the following items listed below. The associated NAICS code for this Procurement is 621511 with a size standard of $36.5 Million in Annual Sales. The Sole Source Contract will be Awarded to: Prometheus Laboratories, Inc. 9410 Carroll Park Drive San Diego, CA 92121-5201 Description of Service Requirements: The Contractor shall Provide the LTC Charles S. Kettle, Ann Arbor VA Medical Center and the Toledo CBOC Facility has requested a Base Plus Four (4) Option Year, Firm Fixed Price Contract. The Test Menu Provided to the Facilities will be varied and is based on the patients test orders that are placed by the Clinicians. Patient specimen transport shall be included in the Contract. The Toledo CBOC currently ships the Patient Specimens for Reference Laboratory Testing to the LTC Charles S. Kettle, Ann Arbor VA Medical Center to process the Patients Specimens for transport to the Reference Laboratory. The Laboratory Test Menu will be based on the Supply and Demand of the Facilities and the Orders will be Issued by the Clinicians. The following is a list of the Testing that would be sent to the Reference Laboratory: TPMT Genetics TPMT Enzymes Theopurine Metabolites Celiac Genetics IBD SGI Diagnostics Once the Patient Testing is Completed, the Patients Test Result Reports shall be faxed directly to the LTC Charles S. Kettle, Ann Arbor VA Medical Center. The Contractor shall provide Reference Laboratory Testing with Reflex Testing at an addition charge and Customer Support Telephone Services 24/7 each year. The Contractor shall provide the Specialized tub es, transport media required for select Patient Testing, Special Packing Materials, Specimen Transport Request Forms, Reference Laboratory Testing and Reporting, Quarterly Turn-Around-Times Reports on Patient Test Results and a Report Delivery via Fax to the Facility. In addition, the Contractor shall provide an electronic and a Paper Test Menu and Pricing Schedule. The Results will be made Available to the Facility PALMS Administrative Office via Fax. A Firm Fixed Price Contract is anticipated and shall be awarded. It is the Government's belief that only the proposed candidate possesses the required capabilities to successfully meet this requirement. It is the Government's intention to Solicit and Negotiate with Only this Source. This notice of intent is not a request for competitive proposals. It is not a solicitation or request for offers. No solicitation package is available. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government.
