COMBINED SYNOPSIS/SOLICITATION� Pest and Termite Control Services ***Similar work was not previously performed under a purchase order, however, see SOW for Termite Vendor information.� Any correspondence email subject line should reference the SAM.GOV Notice ID number: 1333MF22QNFFN0059 �(I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (II) The solicitation number for this procurement is 1333MF22QNFFN0059 and is hereby issued as a request for quotation (RFQ). (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-07 (August 2022) (Deviation 2022-02)(NOV 2021).�� (IV) This solicitation is being issued as a total small business set-aside. The associated NAICS code is 561710.� The small business size standard is $15.5M. (V) This combined solicitation/synopsis is for the purchase of the following commercial service(s): See SF18 CLIN schedule for Base and 4 option periods Non-Personal Services for Pest and Termite Control Services. WAGE RATES INCORPORATED: THE WAGE RATES WD 2015-5183 REVISION 21 ARE INCORPORATED AND MAY BE VIEWED AT SAM.gov� (VI) Description of requirements is as follows:� Non-Personal Services.� The contractor shall furnish the necessary personnel, material, equipment, schedule of treatments, and service to perform provide monthly and as needed maintenance to control pests (annual Termite and monthly fumigation services) at a NOAA Fisheries Laboratory in Lafayette, LA. in accordance with the Statement of Work attached to the Sam.gov combined synopsis/solicitation posting (VII)�Period of Performance: Date of award through 12 months, with four (4) option periods. Place of Performance: Estuarine Habitat and Coastal Fisheries Center 646 Cajundome Blvd Lafayette, LA 70506. Acceptance will take place as deliverables are completed.� Site Visit: All Bidders are strongly encouraged to make a site visit to inspect all proposed work and to acquire all necessary site information to properly prepare the Bid Proposal. Failure of the Contractor to visit the site and acquire all necessary site information to properly prepare the quote shall not be acceptable cause to justify a later request for additional costs after Contract Award. Any questions that come up at the site visit must be provided to Shawana Randolph via email at Shawana.Randolph@noaa.gov. No questions will be answered on site. FAR 52.237-1 SITE VISIT (APR 1984) Vendors are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award. If you wish to have a site visit, an escort is required. Therefore,� Joy Merino, will be your contact for the site visit. PLEASE BE ON TIME AND HAVE ALREADY REVIEWED THE STATEMENT OF WORK AND SOLICITATION. All visitors will be required to sign-in and possibly provide Identification and COVID related documentation.� NOTE: NOT ATTENDING THE SITE VISIT WILL NOT BE AN ACCEPTABLE CAUSE TO JUSTIFY A LATER REQUEST FOR ADDITIONAL FUNDS. IF GOING TO A GOVERNMENT SITE: On April 6, 2022, the Deputy Under Secretary for Operations shared NOAA�s 2022 COVID-19 Reintegration Plan and Community Level Mitigation Measures. As part of this plan, NOAA will evaluate COVID-19 Community Levels and implement appropriate mitigation measures on a weekly basis. These mitigation measures are subject to change and some operational programs may require enhanced mitigation measures.� In counties where the COVID-19 Community Level is LOW, on-site contractors, grant recipients, affiliates, and visitors are not required to provide attestation of their vaccination status and COVID-19 screening is not required. Where COVID-19 Community Levels are MEDIUM or HIGH, on-site contractors, grant recipients, affiliates, and visitors should attest to their vaccination status using the Certification of Vaccination form. Those who are not fully vaccinated or decline to provide their vaccination status, must obtain a negative COVID-19 test dated within the previous three calendar days of entering a Department of Commerce (DOC) operating unit or facility. Individuals must carry their completed vaccination forms and, if required, proof of a negative COVID-19 test results with them while on the federal premises and produce them upon request. COVID-19 viral tests may be any type that has been authorized by the Food and Drug Administration to detect current infection and that documents test results in a written report that can be provided to the individual who was tested. NOAA will use the government-wide community measures to determine the need for face coverings. At this time, government- wide measures include a requirement to wear a mask in worksites where the COVID-19 Community Level is HIGH.� Vendors/recipients must attest, in writing, to the contracting officer or contracting officer�s representative that all of their on-site contractor employees/affiliates are following the updated guidance and the NOAA Community Level Mitigation Measures and are either fully vaccinated or will provide a negative COVID-19 test report dated no more than three calendar days before entering a DOC facility or location. The attestation may take any form so long as vendors/awardees certify that all of their employees/affiliates accessing a DOC facility are in compliance with these protocols. The attestation must be kept in the official contract file. (VIII)� FAR 52.212-1, Instructions to Offerors --Commercial Products and Commercial Services� (NOV 2021), applies to this acquisition. See addendum � ADDENDUM TO FAR 52.212-1 Instructions to Offerors�Commercial Products and Commercial Services� ADDITIONAL QUOTE PREPARATION INSTRUCTIONS: NOTICE TO OFFERORS:� Instructions for submitting quotations under this request for quote must be followed.� Failure to provide all information to aid in the evaluation may result in the offeror being considered non-responsive.� Offers that are non-responsive may be excluded from further evaluation and rejected without further notification to the offeror. Submission of Quotes 1.Submit quotations to the office specified in this solicitation at or before the time specified in the solicitation.� Only Email quotes are acceptable and can be sent to Shawana.Randolph@noaa.gov 2. Contractor shall have an active registration in the System for Award Management (SAM) found at www.SAM.gov in order to provide a quote and be eligible for award. 3. Vendor's response to this Request for Quote shall consist of the documents outlined in the Combined Synopsis Solicitation and outlined in the Contents of a Bid Package included in the SF18.� � (1) The offer must be prepared in two parts: A technical quote and a business/price quote. Each of the parts shall be separate and complete in itself so that evaluation of one may be accomplished independently from evaluation of the other. The technical quote must not contain reference to price; however, adequate information must be contained in the technical quote so that the offeror�s technical approach is sufficiently detailed to provide a clear and concise presentation that addresses all contract requirements and demonstrates a clear understanding of the requirements. �(2) The offeror shall submit: a. Technical Quote: One (1) copy, electronic required. 20 page maximum b. Business/Price Quote: One (1) copy, electronic required. Each part shall be clearly marked by title. There is no page limitation for the price quote. (3) The Government will evaluate quotes in accordance with the provisions set forth in this section and with the evaluation criteria stated below. (4) The Offerors shall identify and explain any exceptions or deviations taken to any part of the solicitation or conditional assumptions made with respect to the technical requirements of the solicitation.� Offerors should note that taking exceptions to the Government�s requirements may indicate an unwillingness or inability to perform the contract, and the quote may be evaluated as such. (b) Technical Quote Vendors shall submit the following in their technical quote:� Factor 1--EXPERIENCE. The offeror's background, experience, and qualifications will be assessed to determine the likelihood that that offeror can successfully perform the contract requirements and the degree of the risk of non-performance. (IX)� FAR 52.212-2, Evaluation�Commercial Products and Commercial Services (Nov 2021), applies to this acquisition.� Offers will be evaluated based on price and the factors set forth in paragraph (a) the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers.�� Paragraph (a) is hereby completed as follows: Evaluation will be based on�� 1) Experience� 2) Price The Government intends to award a best value trade-off firm fixed-price purchase order on an all or none basis with payment terms of Net 30.�� (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer�s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
