Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2022-07, dated August 10th, 2022. Solicitation 36C26222Q1538 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued under Full and Open Competition. NAICS Code is 541519 and the Small Business Administration s (SBA) size standard is $30.0 Million. The Government is seeking a contract to acquire Vault Stream Server and Data center. This solicitation is seeking to fulfill the following statement of work: STATEMENT OF WORK (SOW) Period of Performance. The period of performance shall be for one (1) base year from date of award. Place of Performance. NMVAHCS, 1501 SAN PEDRO DR. SE, ALBUQUERQUE, NM 87108 1. Olympus Vault Stream Server and Data Center 2. Background. The NMVAHCS recently replaced all aging Kay Pentax and other manufacturers scope equipment with standardized Olympus America Inc. laryngoscope and endoscope equipment throughout the medical center. The older equipment was no longer supported and failing. Additionally, the Sterile Processing Service recently adopted new methods of sterilization of this critical reusable medical equipment and the older equipment could no longer be safely sterilized or processed. VISN 22 instituted a Scope Standardization/Replacement Initiative to ensure all laryngoscopic and other endoscopic equipment and processing towers were standardized, fully supportable, serviceable and compliant with current required reusable medical equipment sterilization procedures. All components EXCEPT this Vault Stream Server and Data Center were purchased at the end of FY21 by NMVAHCS Medical Logistics Service via the ECAT system. This equipment item was not listed as available. Meanwhile, this is a critical, proprietary and absolutely essential component of a complex medical system and must be acquired for the storage and transmission of digital still and video imagery of scope studies for use by physicians and other specialty providers. This equipment item is comprised of a central data center or processing unit, with licenses and connections to serve 8 different Olympus scope systems/towers located in the Speech Pathology Department of the PM&R Service, as well as the Ear, Nose and Throat (ENT) Department of the Surgical Service and Spinal Cord Injury Center. 3. Scope. This equipment item is an essential component necessary for the actual operation of the new Olympus Laryngoscope System and Towers purchased for Speech Language Pathology by the NMVAHCS Medical Logistics Service. NMVAHCS is in the process of replacing all older scope systems with standardized Olympus scope equipment as part of a VISN 22 Scope Replacement Initiative. The lion s share of the Olympus equipment has already been purchased by Medical Logistics via ECAT; this particular equipment item, however, was not available to be purchased via ECAT at the end of FY21 with the rest of the Olympus scope equipment. This Vault Stream Server and Data Center is a digital information archive that stores, sends and receives digital still and video imagery of scope studies. 4. Specific Tasks. Requested Equipment Items (component assemblies) will possess the following characteristics: - A smart, centralized content management system that provides secure, HIPAA compliant storage and access for medical case video and images across the hospital.  - Case information, including recorded video and saved images, is automatically stored on the centralized library at the conclusion of each procedure. This enables clinicians to securely share and access case information from the hospital network.   - Access images and videos with a hospital-networked PC or iPad® from wherever the provider is located via intuitive browser interface.  - Enables authorized users to access post-procedure video and images without the need to export patient data to insecure computers or flash drives.  - Side-by-side comparisons can be done between new and old studies to see how a patient s disease stage has changed over time.  - Clinicians who are granted access (approved by hospital clinical staff and IT dept), can view still images and videos for any previous procedure from any hospital-connected computer. a centralized library is created that can be filtered by Clinician, Procedure Type or any other 'tags' that get added to that study. (Such as image and video labels).   - Interoperable and scalable architecture enabling VaultStream to meet the needs of a single O.R., a lab, a department, an entire hospital, or a nationwide hospital system, with a right-sized site-hosted server or virtual machine. The ability to organize large amounts of procedure information in separate libraries with customer retention policies improves cross-department efficiencies in sharing, searching, and storing image data.  - HIPAA compliant protecting patient privacy while supporting clinician collaboration  Named user accounts  Authentication management  Access attempt logging  Secure connection protocols and secure storage  - Scalable, library-based architecture.  Leverage enterprise storage infrastructure for archive and disaster recovery  Configurable case retention policies Digital image capture & recording capabilities with all Olympus towers  Links every Olympus. This can include multiple locations as long as both offices are on the same server.    Saves and provides access to all scope exams/studies captured online to provide for easy access for provider staff.  All data would be saved in a secure on-line portal that is accessible while at the office, or connected to the internet and server through a secured VPN.  4.1 Task 1 - Enterprise Management Controls. 4.1.1 Subtask 1 - Integration Management Control Planning. Provide the technical and functional activities at the required level for integration of all tasks specified within this SOW. Include productivity and management methods such as quality assurance, progress/status reporting and program reviews. Provide the centralized administrative, clerical, documentation and related functions. 4.1.2 Subtask 2 - Contract Management. Prepare a Contract Management Plan describing the technical approach, organizational resources and management controls to be employed to meet the cost, performance and schedule requirements throughout Contract execution. Provide a monthly status report monitoring the quality assurance, progress/status reporting and program reviews applied to this contract. The information identified above is intended to be descriptive, not restrictive and to indicate the quality that will be satisfactory. The interested parties bear full responsibility to ensure their submission demonstrates to the government that they can provide the services being requested. Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the lowest price on an Acceptable/Unacceptable evaluation process. Submitting Quote: Offeror(s) shall submit their quote via email on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm s UEI#, total price on Attachment A and a completed copy of FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items or a copy of offeror s annual offeror representations and certifications (ORCA) document from www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items. 1. Offerors shall fill out all unlocked fill-ins/tabs (define your fill-ins or tabs) of Attachment A conforming to requirements stated by the salient characteristics and the instructions below. a. Offerors shall complete column K of tabs 1 for unit prices. b. Offerors shall additionally complete columns F H of tabs 1 if providing an equivalent product. c. If providing an equivalent product, offerors shall either provide information regarding how their product meets the salient characteristics in Attachment A in Column I or alternatively may state where in the offeror s overall submission that information can be found. CONTRACT CLAUSES AND PROVISIONS 52.212-2 - Evaluation Commercial Items (Nov 2021) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical acceptability shall be established by review of each submitted quote, attachments, and/or information by the designated technical evaluator/s and verification that any brand name or equal to items"" meet all the salient characteristics included in this solicitation. No other rating or quantitative value other than acceptable or unacceptable will be assigned. Once technical acceptability has been established, the Contracting Officer shall award to the lowest price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of clause) 52.211-6 - Brand Name or Equal (Aug 1999) (a) If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation. (b) To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. (c) The Contracting Officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an equal product, the offeror shall provide the brand name product referenced in the solicitation. (End of clause) 52.217-6 - Option for Increased Quantity (Mar 1989) The Government may increase the quantity of supplies called for in the Schedule at the unit price specified. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days. Delivery of the added items shall continue at the same rate as the like items called for under the contract, unless the parties otherwise agree. (End of clause) 52.217-8 - Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days of contract expiration.
