Page 4 of 4 Edstrom Watering Service Agreement Department of Veterans Affairs (VA) Veterans Health Administration (VHA) BRIEF SCOPE OF WORK (This is for information only) The contractor shall provide preventive maintenance and repair services for the Edstrom Watering System used at the Animal Research Facility, VA Medical Center, Oklahoma City, OK. Services are to be performed in accordance with this PWS and to OEM standards, with the intent of ensuring the equipment operates as the original manufacturer intended, uninterrupted (95% uptime), in a cost-effective, efficient manner, contributing to the VA Mission. TASKS and REQUIREMENTS: The contractor shall provide services (Basic Service plan with parts) on Pressure Reducing Stations, Room Distribution System, Cage Wash Equipment and Pulse. Preventive Maintenance Services for designated equipment will be performed twice (2) per year, Annual Service and Interim Service. These include verification, calibrations, adjustments, repairs and/or replacement of worn/defective parts due to normal use and conditions. All Parts are included for service, repair, and/or replacement within this contracted service agreement. The Annual Service visit will be completed within 90 days of contract start date for all equipment detailed on the Scope of work page. Calibrations, adjustments, repairs, and applicable Preventive Maintenance services will be done on covered components. The Interim Service visit will be completed approximately (6) month after the annual visit for all equipment detailed on the Scope of work page. Calibrations, adjustments, repairs, and applicable Preventive Maintenance services will be done on covered components. A Preventive Maintenance report will be completed for each visit and presented to an authorized company representative for approval. An original copy signed by both parties will be given to the representative. 24/7 phone support. All technicians must be Fully Qualified based on successful completion of factory training and experience in the field. For training, the technician or engineer shall have successfully completed a formalized training program for the equipment identified in the schedule. The contractor shall ensure all contractor supplied test equipment is properly calibrated, and certified. If requested, the contractor shall be able to supply any certificates tied to the quality of the test equipment. Test equipment calibration shall be traceable to the National Institute of Standards and Technology (NIST). Vendors are advised to familiarize themselves with Federal Acquisition Regulation parts 19.505(a)(2) and 19.505(b)(1) as it pertains to subcontracting limitations. Vendors are also advised to familiarize themselves with 13 CFR 125.6 and the penalties described in this regulation.
