- The Department of Veterans Affairs Great Lakes Acquisition Center (GLAC) in North Chicago Il intends to negotiate a sole source contract with KCI / BEACON POINT ASSOCIATES LLC. 1216 SW 4TH ST STE 4 CAPE CORAL FLORIDA 33991 at the Hines VA Healthcare Systems at 5000 5th Ave, Hines, Il 60141. This contract will be for a firm fixed price contract. This procurement is being conducted in accordance with FAR 6.302-1, Only One Responsible Source and no other supplies or services will satisfy the agency requirements. The contract will be firm fixed price and the NAICS code is 532490 This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products, services and capabilities as well as references from other customers who are using these products and services provide service for the maintenance and repair of the Fire shutter located at the Hines VA 5000 5th Ave Hines Il 60141 to the contracting officer NLT 11:00AM CST, 22 August 2022. Supporting evidence must be submitted in sufficient detail to demonstrate the ability to comply with the above requirements and attached Statement of Work, to include access to OEM parts for repairs. Responses received will be evaluated; however a determination by the Government to not compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with KCI USA Inc /Beacon Point Associates . STATEMENT OF WORK V.A.C Ulta Therapy Unit Management 1. Requirement: This requirement is for annual service and preventative maintenance of 31 V.A.C Ulta Therapy Units at Edward Hines Jr. Hospital. 2. Background: Edward Hines Jr. VA Hospital is the largest VA in Illinois and treats over 44,000 veterans that include 6 community-based outpatient clinics. Currently the hospital operates approximately 483 beds and offers a wide range of health and support consisting of primary care, mental health, specialty care, social programs, telehealth, and whole health services. The V.A.C Ulta Therapy Units provide negative pressure wound therapy and are vital for Hines VA hospital s day to day operations in wound management therapy. 3. Performance Period: This procurement is for a base Year Plus (2) Two (1) one option years. During normal working hours of 8:00 a.m. through 4:30 p.m., Monday through Friday. Excludes weekends and holidays. No overtime is authorized at this time. 4. Place of Performance: Edward Hines Jr. VA Hospital, 5000 S. 5th Ave, Hines IL, 60141 5. Scope of Work: A Contracting Officer Representative (COR) shall be assigned to this procurement and written notification will be provided to the Contractor after award. Scheduling of work performance shall be arranged through the designated COR assigned. The Contractor shall conduct semi-annual preventive maintenance and inspection of the V.A.C Ulta Therapy Units during the months of April and September each calendar year. The preventative maintenance consists of: Inspection of instill motor hours Verification of Manometer functionality Pressure accuracy test of manometer Using test canister on unit Unit alarm testing for leaks, blockage, canister not engaged alarm, pump duty, V.A.C. VERAFLO Blockage alarm Ensure all checks completed Vendor requirements and utilization of quality control checklist as cleaning, preventative maintenance, quality control, and repairs between patient placements during site visits include: Asset tracking and management of customer owned vs. rental items Cleaning, Preventative Maintenance, Quality Control, and repairs between patient placements during site visits Reviewing retrofit information Battery function verification Verification of labels and power cord placement Verification of power supply is compliant Verification proper components are installed Check power function of the units Verify correct settings such as time, date, and region and matching of 4 therapy units. Accessed Quality Control Checklist Exported logs as needed Upgrade software as needed Determine if PM is due. Attach test canister to the unit Check Pressure accuracy test: Unit has passed when leak alarm has passed, blockage alarm has passed, canister not engage alarm passed, canister full therapy interrupted alarm passed, instill pump performance passed, and V.A.C. Veraflo blockage alarm passed. Check unit for passing of Electrical safety test-as required. Record values or attach printout of leakage current values (with power on), and verification of unit pass of power cord test, if required by national or local protocols. Verification if unit is branch ready and restore to patient defaults, turned off, ensure packing and documentation included, and moved to branch ready. 6. EMERGENCY CALL IN Requirement: The Contractor shall provide a telephonic method for the facility to call in emergency work orders for the system during any failures. Upon notification of failures, the Contractor shall have all repairs made at the vendor Technical Repair Facility. If there is an emergency that would require additional wound VACs to be used over the owned capacity of 31, vendor strives to provide rental wound VACs that are deliverable in approximately 4 hours. 7. Report requirement: The Contactor shall provide a report of service in April and September after all preventive maintenance has occurred for each system. This report shall be submitted to the COR no later than ten (10) business days after each inspection occurs. The report shall contain information described in the vendor quality control checklist described in the scope of work. 8. Check-In requirements: Contractor and all Contractor s service personnel shall check in and out with Central Supply Supervisor and/or Lead Supply Tech prior to performing and upon completion of all required service. This information is required to contact mechanics in case of an emergency during regular hours. Contractor shall follow all COVID-19 safety requirements issued by the facility. 9. Safety Requirements: Contractor shall ensure all tools and equipment are secured at all times. Tools and equipment must be removed from open areas or stored in secured location during breaks, lunch, and/or at the end of each workday. All debris must be removed from the facility upon completion of service each day. All personnel are required to wear approved masks when entering or working in any of the facilities buildings. Masks are to be non-surgical type masks, not homemade masks (this is for staff and Contractors) and to be worn at all times. Any personnel caught not wearing an approved mask will be escorted out the facility. Masks need to be worn when whenever you are within six (6) feet of another person, whether outside or inside and includes riding in carts. Contractor shall ensure proper signage, barriers, and/or blockage of service area is provided where applicable, especially at hoist way doors and clearly visible to ensure patient/employee safety is adhered to. Smoking is not permitted on Edward Hines Jr. VA hospital at any time. All smoking shall be conducted at designated areas outside the facility grounds only. 10. Training requirements: Certification of Training to work on the V.A.C Ulta Therapy Units
