Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation� (RFQ) and the reference number is White River Service Unit. �This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-77 and the Health and Human Services Acquisition Regulation (HHSAR). �The Offeror(s) must quote on an all or none basis. Written quotes are required (oral offers will not be accepted). Award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106-2. Please include CAGE Code, DUNS, Tax Identification Number, FOB Point, Discount Terms and Delivery Date. The NAICS Code is 334516 Size Standard is 1000 Employees. The enclosed solicitation is for any business concerns. To provide moderately complex testing services on a 24/7 basis for the 18,000 patients we serve at Whiteriver Service Unit Laboratory (WRSU). The RPR analyzer instrument provides critical information to the clinician for the treatment of communicably ill patients. A RPR analyzer provides results for syphilis testing. We are requesting a new lease agreement for one RPR instrument, to include reagents, supplies, and consumables. SEE ATTACHED SOW
