Description
39. QUESTION: What was the cost of the final full option year of the previous contract? ANSWER: Base Year - $190,263.00 Option Year 01 - $175,367.00 Option Year 02 - $78,603.00 Option Year 03 - $78,603.00 NOTE: The current contract does have a different Statement of Work. Base Year and Option Year 01 was for transport from Prescott VA and Tucson VA. Contract was modified to remove the Tucson VA transport so cost for Option Year 02 and Option Year 03 was only for Prescott VA transport. The current Statement of Work includes transport to and from Prescott VA and 10 Community Based Outpatient Clinics (CBOC). See Statement of Work for RFQ 36C26222Q0950 for details and locations. 40. QUESTION: How many drivers/vehicles are currently used to service the current contract? ANSWER: (1) One Driver / (1) Vehicle 41. What is the vehicle size currently servicing this location? ANSWER: Tractor Trailer with 53 trailer (Note: this combination will be too large for the new Statement of Work) 42. Section D.1.A. specifies: PVAHCS, Phoenix, AZ. starting at 5:00 AM in the morning Monday through Friday returning from NAVAHCS, Prescott, AZ. by 10:00 AM on the same day of delivery and if necessary, will do a 2nd run one day a week depending upon production. QUESTION: Is the 2nd run referring to the second weekly trip Tuesdays/Thursdays, or is it referring to a second trip on the same day either Tuesday or Thursday between PVAHCS/NAVAHCS? If it means a second trip on the same day, is this a definite weekly occurrence, and so the expectation should be 3 weekly round trips between PVAHCS/NAVAHCS? And is the need for a third trip based on the volume of laundry carts requiring transport? ANSWER: Tuesday s and Thursday s are the Phoenix / Prescott round trip. The if necessary, will do 2nd run refers to i.e. contractor driver does not collect all the soiled or clean on the original trip and would need to go back and / or an emergency situation. (Note: a 2nd run has never occurred to date but again, if driver does not complete it on the first run it may be necessary.) 43. QUESTION: Do the CBOCs require full laundry cart transport? If not, what is the clean/dirty linen volume being transported between PVAHCS and each/all CBOCs? If not carts, how is the linen transported? ANSWER: Yes, and laundry carts are utilized 44. QUESTION: Do you accept CPARs evaluations as additional Past Performance information? ANSWER: See Section E.5 Addendum to 52.212-2 Evaluation Simplified Acquisition Procedures Evaluation Approach (2) Past Performance 45. QUESTION: Regarding specific employees information: It is common practice for small businesses that the drivers, cars and other equipment used for a contract are not sourced until after an award has been made, for cost effectiveness and to reduce loss. However, we will be forwarding our standard Training Certificates-Procedures for your reference, is this information acceptable? ANSWER: See Section E.1 52.212-1 Instructions to Offerors-Commercial Products and Commercial Services Content Organization for the documentation that needs to be submitted with quotes. 46. QUESTION: Regarding evaluation factors for the price Volume: we usually like to provide a Pricing realism cost break-down, to include SCA minimum per year, the number of drivers, hours and miles that are estimated to be required for performance, and other costs. Is it acceptable that we forward this Pricing Realism chart with our Price response? ANSWER: See Section E.5 Addendum to 52.212-2 Evaluation Simplified Acquisition Procedures Evaluation Approach (3) Price
