- STATEMENT OF WORK 2237 Funding/ General Equipment Name:� 636-22-4-3998-0306/ Virtual Mobile Clinic (VMC) Van THIS IS A BRAND NAME OR EQUAL (BNOE) REQUIREMENT FOR FARBER MH-38 VIRTUAL MOBILE CLINIC VAN Description:� VA-CIHCS Virtual Health has an immediate requirement for: one (1) Virtual Mobile Clinic (VMC) vehicle to support outlying remote and rural locations in the VA-CIHCS area for providing telehealth and clinical (primary/ specialty/ mental health/ surgery) services to eligible Veterans.� The VMC is to be a fully independent stand-alone clinic for year-round operation.� The VMC must be able to be operated without a Commercial Driver license (CDL), self-contained, all weather, three-room configured, with on-board 20KW (minimum) stand-alone power source, and running hot/ cold water with on-board toilet facilities.� The vehicle will be fully compliant with current ADA guidance, with a patient wheelchair lift for full accessibility to the clinic area(s).� The VMC will be fully equipped with clinical furnishings for each patient exam area.� VA-CIHCS Extended Care and Rehab (ECR) Virtual Health is seeking a name brand or equal purchase of one Farber MH-38 (MH) Mobile Medical.� Award must be for not later than 30SEP2022, to allow for the long production lead-time to produce and deliver (300 days ARO).� Salient Characteristics (at least 5), The item must: be/ have/ do these things The Vehicle Chassis must: Be gasoline powered Be able to be operated without a CDL for vehicle operator Have seating for driver/ operator and front passenger Have two (2) additional DOT rated seating in coach body/ clinic area Be accessible between the coach and vehicle operator areas Have Cruise Control Back-up/ safety camera with monitor (not less than 7 inch) The Coach Body must: Have three main rooms (two patient exam, one reception/ wait area/ lab draw) Powered Slide out room (12 feet in length by 24 inches deep) Powered awning of 12 feet in length or greater Have cabinetry and surface counter space adequate for exam and reception/ lab areas Have adequate climate control (air conditioning and electric heat) for use in the upper Midwest to maintain interior coach temperature (45 to 85 degrees Fahrenheit thermostat) with remote monitoring. Have running (hot/ cold) water and supporting monitoring panel, pump(s), water storage, hot water heat (electric), and holding tanks (grey/ black) Have an ADA compliant lavatory (with marine toilet, sink, basin, ventilation fan, hand railing, coat/ clothing hook) Have a sample/ specimen pass through (door) from Lavatory Have a cold weather package that seals the underbody and water storage area(s) for year-round operation (for Iowa winters) Be able to be stored outdoors year-round Have temperature monitoring for the coach and underbelly water storage area(s) Window must be tinted and/ or have interior window treatment(s) for privacy Have coat hooks for three rooms (x2 exam, lab/ waiting) Exterior ADA approved steps Have an automatic ADA approved wheelchair lift with door (1000lb weight, 34� wide minimum).� meet or exceed specifications of Braunability Century 2 series lift Hydraulic levelling/ stabilizing vehicle jacks Provide (3) television (32inch minimum) and wall mounting bracket (1 per exam (2 total), and 1 in intake/ waiting area) Microwave oven (cabinet mounted, 120v) Antenna mounting point/ rail for exterior antenna(s) with weatherproof access to interior (for VA provided hotspot) Exterior wrap/ graphics allowance Have three (3) minimum rear speakers from cab area AM/FM/ CD stereo Have one (1) cabinet mounted microwave oven Have one (1) refrigerator (AC/DC, compressor operated, 3.0 CF minimum, with freezer compartment, RV latches) Have three (3) minimum roof mounted ventilation fans Have three (3) minimum reversible roof mounted air conditioners with covers Have interior LED Lighting Have a vehicle security alarm system (with motion sensor(s)) Provide interior electrical outlet plan for entire coach space Have at least one (1) exterior GFCI outlet Exterior belly storage compartments Operator training for system(s) (in person, upon or following delivery) Delivery to 3600 30th Street, Des Moines, Iowa, 50310 The Clinical Furnishing/ Medical Equipment Required must:������ Have two (2) Otoscope, Ophthalmoscope, specula dispenser, thermometer, wall aneroid (Welch Allyn777/ equal to/ better)� Have one (1) scale, (digital, wall mounted head, 500lb capacity) Have two (2) exam tables (Ritter 204/ comparable) Have three (3) physicians wheeled adjustable height stools with back Have one (1) blood draw chair/ bench Have one (1) Medical-grade refrigerator, 5.5 CF Minimum (specifications attached for Fisher Brand FBG505SA under counter laboratory grade refrigerator) Have a dedicated �L� shaped reception counter/ work surface area in intake area near entry door All cabinets and working surfaces will be durable for clinical use and cleansing Each patient exam space/ area will have one each locking cabinet and drawer The Power Generation (stand-alone) must:�������� Be remote stop and start Be gasoline powered Be 20KW or greater Be water cooled Compartment installed Be able to run off the main vehicle fuel tank (no second fuel tank needed)��������� VA provided equipment items:� The VA will be transferring from existing mobile clinic/ procuring new the following items.� VA will coordinate with vendor on installation or install instructions for awarded vehicle: Wheelchair scale (folding/ fold-up) Portable AED Portable eyewash Installation of 4G RV internet hot spot and router antenna (roof-top), Planning on Winegard model # Connect 20.4G2, WF2-435 mobile hot spot ( https://winegard.com/connect-2-4g2/ ), and router.� Paper towel dispensers, hand sanitizer dispensers, Sharps containers, sink for each room (3) Evaluation Criteria (Best Value) Technically acceptable (meets or exceeds salient characteristics of BNOE Farber MH-38) Small business consideration Cost Required Individual Line Items & Quantity of Items Item # Description/Part Number* Qty 1 Turn-Key three room Virtual Mobile Clinic (VMC) Vehicle 1 �(1 each) Maintenance Manuals (CD, PDF or Word) (1 each) User Manuals (1 each) Cleaning Instructions for all equipment, supplies and accessories 1-year warranty included in cost (parts and labor) Optional Line Items Trade-In Items (include make, model and serial # of all items to be used as a trade-in) �*Consult Logistics � no trade in for this requirement.
