- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) Unique Entity ID (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The Northern Arizona VA Health Care System (NAVAHCS) located at 500 Highway 89 North Prescott, AZ 86313 is seeking a potential qualified contractor that can provide . Certification and Calibration of all pallet jacks, scale jacks, electric jacks and forklift. Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Business set aside. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 811219 ($22.0 Million). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit information by e-mail to Jeremy.seaton@va.gov. All information submissions to be marked Attn: Jeremy Seaton, Contract Specialist and should be received no later than 12:00 pm PST on August 31, 2022. NAVAHCS-Prescott Draft Statement of Work Certification and Calibration of all pallet jacks, scale jacks, electric jacks and forklift Service being ordered: NAVAHCS has an annual requirement to load test, certify and calibrate all pallet jacks, scale jacks, electric jacks, and forklifts. The C&A requirements do not apply, and that a Security Accreditation Package is not required. Salient Characteristics of Equipment Required: EE25600 Liftrite scale/jack - calibration/certification - XX EE32987 Dayton manual pallet jack certification - XX EE25602 Electric Scissor lift jack certification - XX EE13591 manual jack certification - XX EE16976 Toyota electric jack certification - XX EE34787 Raymond electric jack certification - XX EE34788 Raymond electric jack certification - XX EE32361 Octane Forklift certification - XX EE33427 Doosan Forklift certification - XX EE17349 Hyster Forklift certification - XX Specific Requirements The contractor shall provide the load test and either pass or fail at the time of the test. The selected contractor shall be certified by the state of Arizona as qualified to provide certification and calibration services. Contractor Personnel shall be readily available at the site during the entire calibration and certification process. All pieces of equipment that pass shall be certified as so with the company name, phone number, inspection date, rated capacity, make, serial number, inspector name and an expiration date of 1 year from the date of testing. All Services defined in this agreement shall be limited to the following equipment as listed in Section 2: pallet jacks (2), manual pallet jacks (6), electric pallet jacks (2) and 1 forklift. The contractor will provide all needed weights and materials to accomplish calibration and certification on 2 scale jacks and certification of all other listed pallet jacks and forklifts (11 pcs total) NOTE: There is one (1) pallet jack with a 10,000-lb. capacity so extra weights are required for it. General Information The contractor will be responsible for checking in with the VA Police and receiving a temporary ID badge to perform the requirements of the contract onsite. Removing objects from refuse for personal use is unauthorized. Contractor Personnel must adhere to standards of conduct as established by the Facility Director. Contractor shall adhere to current installation security policies. Records The contractor shall be responsible for creating, maintaining, and disposing of only those government required records that are specifically cited in this SOW or required by the provisions of a mandatory directive listed in. If requested by the Government, the contractor shall provide the original record or a reproducible copy of any such record within five working days of receipt of the request. Develop and provide at the start of the orientation period or the start of the first operational performance period (if there is no orientation period) a safety plan for the protection of government facilities and property and to provide a safe work environment for contractor personnel. Government Furnished Property (GFP) and Services The US Government will not provide any equipment, fuel, personnel, utilities or facilities to the contractor for the execution of this contract. Should an emergency arise, only the necessary basic life-saving and life support measures will be provided during the emergency. Hours The posted hours of operation for the Warehouse, Bldg. 20, is from 7:30am 3:30pm
