- This is a notice of intent to award a sole source contract and is not a request for quotes. Department of Veterans Affairs, VISN 2, Stratton VA Medical Center intends to award a Firm Fixed Price (FFP), brand name only, sole source action to: Name of Contractor: Laerdal Medical Corporation Street Address: 167 Myers Corners Road City, State, Zip: Wappingers Falls, NY 12590 This procurement is for Nursing Education to purchase Simman Training Simulator. The Veterans Health Administration prohibits the use of gray market items in their Medical Centers. Gray market items are items outside of the original equipment manufacturer s intended distribution channels. The Education department at Stratton VA has strategically set up the training and education of staff with Laerdal products to streamline and simplify training using this specific equipment the staff is used to it will eliminate training and patient care errors. List of Products: Common Nomenclature: Sim Man 3G PLUS Extended Warranty 1 Year Extended warranty includes materials, manufacturing or workmanship issues, return to bench service only, packaging materials for return to Laerdal, shipping costs for repairs (Ground freight from/to customer's facility). Training must be scheduled with-in sixty (60) days after product delivery and completed with-in one (1) year after product installation. Delivery: Albany Stratton VA Medical Center, 113 Holland Avenue Albany NY 12208-3410 This section should clearly state: Contractor must deliver all components for fully operational system. Contractor must deliver all components of the system as stated in this solicitation within 30 days of award notice. Contractor must deliver all items in this solicitation to - Upon receipt, the Government will inspect items for condition and working operation and acknowledge condition within 1 week of receipt. Standard FOB Destination is required Warranty: Products shall be warranted to be free from failures due to defects in materials or workmanship to include parts and labor; 4 years (48 months). This notice of intent to sole source is not a request for quotes; however, all responsible sources may submit a capability statement which shall be considered by the Government. All capability statements must be received by no later than 3:00 PM Eastern Standard Time on August 31, 2022. Responses to be submitted to Chad M. Johnson, Contracting Officer at chad.johnson4@va.gov. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.
