This announcement constitutes the only Solicitation; proposals are being requested, and a written solicitation will not be issued. The solicitation number 36C24922Q0609 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06. Item: Blood Bank Reagents, the NAICS Code for this request for quotes is 334516 (1,250 Employees). No set-aside is used for this Solicitation. The products being requested are for the laboratory testing of patient specimens. Services will be performed at the Lexington VAMC. This is a Firm Fixed Price BPA contract with a base year and four (4) option years. The Period of Performance is as follows: Base year: 10/01/2022 09/30/2023 Option year one: 10/01/2023 09/30/2024 Option year two: 10/01/2024 09/30/2025 Option year three: 10/01/2025 09/30/2026 Option year four: 10/01/2026 09/30/2027 Questions concerning this Solicitation will be sent to Contract Specialist James Moore at james.moore20@va.gov, NLT 1500pm CST on 31 Aug. It will be at the Contracting Officers' (CO) discretion to address any questions after that time. Submission of Quotes: Vendors interested in providing the required supplies will need to submit all required documentation to Contract Specialist James Moore at james.moore20@va.gov detailed in Appendix B Blood bank Reagents (Blood Cells) below, NLT 1500pm CST on 02 Sept. Late quotes, or documentation will generally not be accepted, however, it is at the discretion of the CO to accept late quotes. Please reference the following sections for more information. Section B.2 Items/Price Cost Schedule and Delivery/place of performance SOW included in Solicitation Appendix A Blood Bank Reagents (Blood Cells) Evaluation Criteria Appendix B Clauses and Provisions Appendix A Blood Bank Reagents (Blood Cells) Evaluation Criteria The following factors will be evaluated as the basis for an award. Three volumes will be submitted when submitting quotes: Price, Technical, and Delivery. Ensure all volumes are in .pdf format. Evaluation Criteria: FACTOR 1: PRICE Provide prices for all line items listed in Appendix A in the Price/Cost Schedule. Prices shall not exceed those listed in Respective FSS contracts. FACTOR 2: TECHNICAL 1. The Contractor shall provide the following documentation with their offer: a. Response to the Solicitation, including the pricing for all line items listed in the Price/Cost Schedule. b. Proof of Valid FDA Registration/Licensing. c. Valid Quality Management System Certification. d. Link to catalog or services manual that provides a list of reagents/supplies. (We have the list included on the GA schedules) FACTOR 3: Delivery availability Must be able to meet delivery availability and requests made by the customer. The first POP begins 10/01/2022. All potential vendors' past performance and responsibility will be reviewed. We will utilize SAM.gov, Contractor Performance Assessment Reporting System (CPARS), and Duns and Bradstreet Financial reports to help determine contractor responsibility. However, the CO reserves the right to use whichever means considered necessary in these determinations. Appendix B Clauses and Provisions The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation criteria are technical acceptability and price. Under technical includes the ability to provide all of the requested items in accordance with the Statement of Work, provide a letter of authorization (if not the manufacturer) and a description of equal items, and have, at minimum, a ""Satisfactory"" past performance rating. Offerors are to provide a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions- Commercial Items, applies to this acquisition. There are no addenda to this clause. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
