Description
Statement of Work Audio Visual (A/V) Quarterly Preventative Maintenance Service 1. INTRODUCTION: The purpose of this procurement document is to maintain the Award a service contract, base year plus 4 option years for the maintenance of the three (3) Audio/Visual systems located in Rooms, 204, Atrium, and 215 in Bldg. 46 and the training room on the 3rd floor of Bldg. 47 at the Bay Pines, FL, VBA St Petersburg Regional Office in a fully operation condition, performing all required preventative maintenance and upkeep thus ensuring system compatibility. It is essential the audio-visual systems are fully functional to ensure employees can maintain training efficiencies and ensure the management team can communicate to the organization as needed. 2. PERIOD OF PERFORMANCE: The period of performance will be for one year from date of awarded contract with four option years based on satisfactory past performance evaluations. The first PM will need to be performed within 10 business days of award. 3. HOURS OF OPERATION All work will be performed during normal operation hours: Monday-Friday 7:00am-4:00pm. Any work that would cause disruption in routine business operations must be performed after hours and must be scheduled with the COR. 4. HOLIDAYS: No work will be performed on federal holidays or other mutually agreed upon time. The observed holidays include New Year s Day, Martin Luther King Day, Juneteenth, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. All services must be scheduled and coordinated with the Contracting Office Representative (COR). 5. LOCATION: All maintenance visit locations and address: St Petersburg VA Regional Office, BLD 46, & 47 9500 Bay Pines Blvd, Bay Pines, FL 33744-8700. B-1 Training teleconference Room 215 (A, B and C), Room 204, Atrium, and 3rd floor training room Bldg. 47 6. SCOPE Of WORK: The Scheduled preventative maintenance, to be performed monthly, will include: Visual inspection of components for wear or damage. System performance analysis. Main power supply inspection. Cleaning of all control components. Perform testing, calibration and analysis of all installed A/V systems as prescribed by the manufactures specifications. Checking all system control panels/circuit boards to ensure system integrity, free of dust/debris, lose or faulty wiring. Adjust, if necessary, all audio equipment associated with installed A/V equipment for distortions or static. Test all recording and listening components for proper operation. Inspect all cables, to include electrical wiring, for loose connections, fraying and general wear. Replace if required Test all projectors for manual or auto focusing. Inspect all controllers, whiteboard, and retractable screens for proper operation. Ensure all A/V equipment is free and clean of dirt/dust and other debris. Inspect for color quality. Lubricate any moving parts that may be required for proper function and operation. Checking data logs. Perform any required test and logging the results. Perform microphone systems check. If found, reporting any current damage or components that are likely to fail. 7. IDENTIFICATION: Contractor s employees must have identifiable information on their uniform, to include their name and company s logo at a minimum. 8. SERVICE VISIT/CALLS SIGN-IN: Service visits will be mutually coordinated with the COR and contractor. The Contractor will sign in and out while performing any service. An escort will be provided throughout the maintenance service visit to ensure established RO security requirements are upheld. The COR shall provide instructions as to the point of contacts and locations of sign-in/out. All service visits/calls shall be documented. 9. REPORTS: Following the preventative maintenance visit, the Contractor shall provide a written report to the Contracting Officer s Representative (COR) within ten (10) business days, detailing the results of the maintenance visit, noting, and addressing any discrepancies found and making specific recommendations toward future remedial action, or upgrades. Any unsafe conditions encountered during the service call shall be reported immediately to the COR, advising the Department of Veterans Affairs of the found condition(s). 10. INVOICE / BILLING Invoices will be submitted via Tungsten, an electronic payment method established to streamline the payment process. 11. POINT OF CONTACT: The Contracting Officer shall delegate the COR to monitor the Contractor s performance. SYSTEM FOR AWARD MANAGEMENT (SAM): Federal Acquisition Regulations (FAR) require that vendors register and maintain active registrations in both the System for Award Management (SAM) database (http://www.sam.gov) and VETBIZ (https://www.vetbiz.va.gov) prior to submitting quotes and during the entire period(s) under contract. Award cannot be made until the Contractor has completed registrations. 13. Security SECURITY - The Contractor is required to enter and exit the Bay Pines, FL through the front entrance.  Entering or exiting the building through any other door will require VARO approval and an escort will be required.  Upon entering the building, the contractor and/or contractor employees will be required to show proof of identity (must have a valid photo ID) as well as pass through a security screening. The Audio/Visual technician will always be escorted at all times while inside the VARO. If the contractor cannot present evidence of a favorable background investigation for the technician the COR for this service contract will assist the contractor in acquiring the background investigation, however the contractor is responsible for the cost of the investigation. In addition, a VA Form 20-0334, ANNUAL CERTIFICATION OF VETERAN STATUS AND VETERAN-RELATIVES must be completed by the Contractor, contractor employees, and sub-contractors and original copies turned in to the COR prior beginning construction
