- This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W912J7-22-Q-0002 is being issued as a request for quotation (RFQ). This solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2022-07. This acquisition is set-aside (total) set-aside for small businesses under NAICS 339115, with a size standard of 1,000 employees. Description of Requirement CLIN 0001: TS-610 - Marco Table-Top Digital Refractor Qty: 1 EA -2�x2� footprint -includes all charts for both near and far vision measurements -Allows ambidextrous operation -~10.4�� color LCD touch screen -Ability to save frequently used charts -Programmed monocular fogging -OPD Wavefront ,aps -Forehead test range of 25+/-2mm Shipping shall be FOB Destination. Please include delivery lead times in your quote. The provision at 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition. Addenda to this provision are included in the provision/clause attachment to this solicitation. The provision at 52.212-2, Evaluation-Commercial Products and Commercial Services, is incorporated into this solicitation with the following evaluation factors: Compliance with the specified salient characteristics Price Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services, with their offer unless the applicable representations and certifications have already been completed in the System for Award Management (SAM). The clause at 52.212-4, Contract Terms and Conditions-Commercial Products and Commercial Services, applies to this acquisition. Addenda to this clause are included in the provision/clause attachment to this solicitation. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Products and Commercial Services, applies to this acquisition.
