- Statement of Work Vocational Rehabilitation Service Forklift Temple VA Project Introduction/Background The Central Texas Veterans Health Care System (CTVHCS) requires the purchase of Two (2) Toyota Gasoline Forklifts model 8-FGU25 (Brand Name or Equal) for CTVHCS located at 1901 SOUTH First St. In Temple, Texas 76504 and one in 4800 Memorial Drive, Waco, Texas 76711. Objectives This forklift is needed for Temple Campus recycling center and Waco recycling center. There is no need for training on this equipment due to this equipment has been used previously at this facility. Scope of Work The vendor will deliver items to 1901 S 1st Street Building 206;Temple Texas. 76504 Equipment information (A.) Invoices and Payment Payment to be made monthly, in arrears, following satisfactory performance by Contractor s personnel and upon receipt of a properly prepared invoice. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction. Time period being invoiced for. Description of services performed. Unit Cost billed. Extended amount due. Invoice number, date, and. Contract and obligation numbers.
