General Requirements: Brand name or equal Patient Monitoring System are needed for the Amarillo, TX in accordance with the following requirements: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 10.00 EA $00.00 $00.00 CAREVIEW FIXED/PORTABLE HARDWARE 0002 14.00 EA $00.00 $00.00 SMART MOUNT FOR PORTABLE/FIXED HARDWARE 0003 24.00 EA $00.00 $00.00 INFRASTRUCTURE 0004 1.00 EA $00.00 $00.00 PROGRAM MANAGEMENT 0005 1.00 EA $00.00 $00.00 SHIPPING & HANDLING 0006 10.00 EA $00.00 $00.00 CAREVIEW SOFTWARE LICENSE GRAND TOTAL $00.00 Statement of Work General Guidance Title of Project: Virtual Patient Fall Monitoring and Fall Prevention System Background: As part of the Department of Veteran Affairs (VHA) patient safety initiatives, the Amarillo VA Healthcare System (AVAHCS) in Amarillo, Texas, currently employs traditional patient fall prevention mechanisms to reduce the incidence of patient falls in the acute care unit and an Intelligent Video Monitoring System in the Community Living Center. The Intelligent Video Monitoring System utilizes video cameras in the patient s room combined with sophisticated virtual motion detection software to indicate the likelihood of an upcoming patient fall. The AVAHCS is pursuing adding the Intelligent Video Monitoring System to the inpatient Medical/Surgical unit with the remote monitoring done by the Community Living Center monitor technician. The use of this System has reduced the number of falls in the Community Living Center over the last year, and it is expected to reduce the number of falls in the inpatient acute care unit, thus increasing overall veteran safety. The System consists of several technological components used to monitor the veterans and predict falls. The System includes a physical component that the Contractor will install in the veteran s room, at the monitoring station, and in the server room or communications closets. In the room, the System includes a specialized computer manufactured by the Contractor called the Room Control Platform(RCP), an attached camera that is mounted on the ceiling aligned with the middle of the bed, and an infrared receiver that is attached to the RCP and mounted either on the ceiling or on the frame of the television. The System provides a touchscreen monitor at the monitoring station, which allows nurses to visualize the veterans in their rooms. The Contractor will provide the necessary servers, cabling, and related equipment. Scope of Work-The Contractor will provide, install, and configure an Intelligent video monitoring and fall prevention system to monitor designated patient bed areas to reduce the number of patient falls and increase the overall safety of the AVAHCS veterans. This System will be installed in the AVAHCS Thomas E. Creek VA Medical Center in Amarillo, Texas. The Contractor shall install the system patient bed as those beds become available, and AVAHCS will coordinate bed availability with the Contractor. The Contractor shall deliver the following equipment to include providing to install and make the virtual fall monitoring system full functional: Contractor shall coordinate the installation schedule with the Contracting Officer s Representative (COR). Set-up and Configuration: All equipment shall be fully functional before acceptance by AVAHCS. System Networking Equipment: All servers and equipment necessary to integrate all individual units into a standard system, including all cabling and interconnectivity for a stand-alone system, residing on AVAHCS s existing Engineering network Cabling to TV: The Contractor shall provide all necessary cabling, including connections between various floors, nursing units, etc., to make the System complete and independent from other existing VA networks. All required equipment shall be fully installed by the Contractor, including painting, fire caulking, patching of walls, and other barriers penetrated by the Contractor. The Contractor shall be liable for any damage to VA property during installation or removal of Contractor owned equipment at no cost to the VA (this may include, but is not limited to: drywall/replastering to appropriate finish/paint with matching paint identified by paint scheme from interior design, replacement of damaged ceiling tiles). The Contractor shall stage equipment installation by ward/room based on bed availability as determined by AVAHCS. The Contractor shall coordinate scheduling with the COR; in-tum, the COR will obtain internal approvals with the Nursing Service and other AVAHCS representatives as determined necessary. The installation will be scheduled based on the least disruption to patients and staff. Noise levels are to be controlled to a minimum to not disturb patients. Contractors shall not touch or move patients belongings or medical supplies/equipment. Salient Charaterictics for Hardware: HD Camera, and Smart Mount; Wi FI Enabled; High-Fiedlity Two (2) way Audio; One (1) Way Audio with Multi-Language Support; Two (2) way Video. The Department of Veterans Affairs is seeking both manufacturers and/or distributors of the exact match or functionally equivalent equipment/ services to the following listed above. 52.211-6 Brand Name or Equal. As prescribed in 11.107(a), insert the following provision: Brand Name or Equal (Aug 1999)       (a) If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation.       (b) To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must-            (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation;            (2) Clearly identify the item by-                 (i) Brand name, if any; and                 (ii) Make or model number;            (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and            (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications.       (c) The Contracting Officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer.       (d) Unless the offeror clearly indicates in its offer that the product being offered is an equal product, the offeror shall provide the brand name product referenced in the solicitation. (End of clause) VAAR 852.212-72 Gray Market Items: a) Gray market items are Original Equipment Manufacturers' (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical equipment only for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM, such that the OEM's warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. (End of clause) FAR 52.212-2 Evaluation-Commercial Items: A copy of the authorized distributor letter from the contractor to verify that you are an authorized distributor of the products/services SHALL BE SUBMITTED WITH THE QUOTE AND IS MANDATORY.
