Description
Statement of Work Department of Veterans Affairs (VA) VA Connecticut Healthcare System Annual Cross Connection Public Water System Survey and Backflow Preventor Testing and inspection The purpose of this contract is to establish an Annual Cross Connection survey and Backflow Prevention Device testing for the public water systems located at Newington VAMC facility. A contract shall be awarded for a base year (12 months) with one (1) annual option year to be exercised at the discretion of the government. Facility Information: Newington VAMC 555 Willard Ave Newington, CT 06111 Location: Number of Valves: Building 1 4 Building 2C 5 Building 2E 11 Building 2W 3 Buildings 3 9 Building 6 2 Building 8 2 Building 11 1 Building 12 1 Building 32 1 Building 42 1 Building 65 5 Total Number of Valves: 45 Scope of Work The Contractor shall provide all labor, specialized services, and travel necessary to provide an Annual Cross Connection survey for the public water systems located at the facility. The backflow preventers must be tested as a part of the service. Services shall be provided during normal working hours between 7:00 AM to 4:00 PM EST, Monday through Friday. Service dates and times shall be coordinated between VA and contractor at the convenience of the government. All work shall be performed and completed on a schedule that will be approved in advance. Dates and times of repairs shall be coordinated between the contractor and Government to minimize impact to clinical operations. All work shall be performed by individuals certified by the Connecticut Department of Public Health as Certified Cross Connection Survey Inspectors and Backflow Device Testers and within the drinking water regulations of the Connecticut Department of Public Health. When deficiencies are found, the contractor shall plan to address all repairs needed. The contractor shall submit a quote for any repairs. Please provide unit costs, as well as labor cost, for the most commonly identified deficiencies found during similar inspections in an itemized format per line item. The Government will evaluate the quote for reasonableness upon submission. Should the Government find the costs unreasonable, they reserve the right to contract for this work elsewhere. This shall only be applicable to the Base Year. Documentation: Cross Connection Survey and Backflow preventor device reports shall be submitted to the CTDPH in formats prescribed by the CTPDH. Documentation shall be provided to the VACT Facilities Management Service representative at the conclusion of the inspection and be maintained by the Facilities Management Service and the vendor. Interference to Normal Functions: When the contractor s work interferes with hospital functions, such as when work produces excessive noise, odors, dust, utility service interruptions, or other interferences with normal hospital operations that cannot be contained within the area of work, the contractor shall schedule said work at other than normal hours and as directed by the VA designee at no additional charge to the Government. Debris Removal: The Contractor/Installer shall be required to remove waste materials and debris daily from the campus. All waste material and debris shall be removed off the site by the contractor and shall be disposed of in accordance with applicable Local, State and VA and Federal regulations. Property Damage: The Contractor shall take all necessary precautions to prevent damage to any government property. The Contractor shall report any damages immediately and shall assessed current replacement costs for property damaged by the contractor unless corrective action is taken. Any damaged material will be replaced in a timely manner or corrected by the Contractor with like materials at no extra cost to the government upon approval of the Contracting officer. Additional Information: The Department of Veterans Affairs is a smoke free environment and smoking is not permitted at the facility. . Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
