- THIS IS A SOURCE OF SOURCE NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. Market Research is being conducted to Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Small Business. All other socio-economic groups may provide a capability statement. The Department of Veterans Affairs Medical Center (VAMC) is trying to determine whether to set-aside an acquisition to provide Canteen floor Sink Replacement located at: VA Mid-Atlantic Health Care Network, 1970 Roanoke Boulevard, Salem VA 24153 DESCRIPTION This requirement is to provide repair services to the Canteen floor Sink Replacement located in building 4, room 40B, Canteen Dish Washing Room, at the Salem VA Medical Center REQUIREMENTS Background: The Canteen Dish Washing Room s floor sink has failed and appears to be collapsed. The Salem VA Medical Center is requiring this floor sink be replaced with new material and the floor to be cut, removed, and patched back to match existing floor finishes. Project Scope: The contractor will need to cut and remove flooring to access floor sink and sewer utilities. The contactor will be required to replace the floor sink and attached utilizes and bed piping and sink back and pour and patch back the concrete and apply matching floor coating for a finished product. 3. DETAILED INSTALLATION REQUIREMENTS: The Contractor shall: Dish Washroom work area will be required to have barriers installed in compliance with the Salem VA Medical Center s ICRA policies. Dish machine and sink will need to be relocated to facilitate the cutting of the floor. Floor cutting, removal, doweling, and concrete pouring shall be done after hours to facilitate canteen operation. Floor shall be cut using wet cut method to minimize dust and debris. Only as much floor shall be cut that is needed for the repair. Floor sink and piping shall be replaced and bedded to insure proper drainage, placement, and level bedding for concrete. Vapor barrier shall be placed back under slab with a 2 overlap and consisting of 15 mil plastic. Rebar shall be doweled back to match existing rebar layout with a minimum of 12 insertion with epoxy. Concrete shall be poured back to match existing concrete. Floor coating shall be a matching color, type, and texture. All means will have to be taken to provide a proper cure before area is turned back over to the Medical Center. The NAICS 238220 is applicable to this acquisition; with a size standard of 16.5 Mil. In response, please include the following: A positive statement of your intent to submit a quote as a prime contractor (capability statement) If any teaming or subcontracting possibilities are contemplated, please provide subcontractor s name and breakdown of labor and its performance cost Evidence of experience in work similar in type and scope to include contract numbers, project titles, award dollar amounts, points of contact and telephone numbers Past performance references with points of contact and phone numbers Provide FSS/GSA Contract # if applicable.
