- Description
- 1. The contractor shall provide intraoperative laser support services that encompass the provision of medical laser systems and medical laser systems specialists to operate the laser systems. 2. Services will be ordered on an as needed basis; whereas the Government will provide a list of scheduled procedures to the contractor as least fifteen (15) days prior to the requirement for service. 3. The contractor shall set-up and operate both Government furnished and contractor furnished medical lasers in an operating room environment and provide full support for requested laser services indicate below. Full support is defined as providing the laser (if needed) and a technician to set-up, break down, and monitor equipment and required consumables. 4. The contractor must have the ability to provide full on-site support with a minimum of twenty-four (24) hours� notice. 5. The laser systems shall provide support for the following medical procedures (this is not an all-inclusive list): Colposcopy Condyloma ablation Dermal ablation Laparoscopic laser ablation Lithotripsy Stapedectomy Tympanomastoidectomy Vein laser therapy Vulva ablation The MTF requires Intraoperative Laser Support Servcies in their Main Operating Room (MOR). The service requirements encompass the provision of medical laser systems and medical laser systems specialists to operate the laser systems. Service will be ordered on an as needed basis; the Government will provide a list of scheduled procedures to the contractor at least fifteen (15) days prior to the requirement for service. The contractor shall recognize that the schedule provided may change within those fifteen (15) days. The contractor must have the ability to provide full on-site support with a minimum twenty-four (24) hours� notice. The Government reserves the right to request services in alternate treatment rooms located within the MTF proper.� The contractor shall provide laser systems specialists to support laser procedures in the MTF MOR. The laser systems specialists shall provide support for the following medical procedures. This is not an all-inclusive list. The MTF MOR may request intraoperative laser services for other procedures as deemed necessary by a provider. The contractor shall perform a general pre-procedure inspection of government owned laser systems for approximately 40 treatment sessions per year and inform the government of any discrepancies prior to use of the affected laser system. The contractor shall not attempt to repair any government owned equipment. The contractor shall provide laser operators who are certified and/or qualified to operate the lasers listed in this document, as well as the lasers in the MOR. The laser equipment used in the MTF MOR is as follows: Lumenis Versapulse Powersuite�Dual 80/100W. Laser for urology procedures, 2100 nm and 1060 nm wavelength, power output up to at least 100W. Lumenis Ultrapulse Encore. CO2 laser for dermatologic procedures, 10,600 nm wavelength, power output up to 60W-100W. Ultrapulse Encore Surgical. CO2 laser for surgical/dermatologic procedures, 10,600 nm wavelength, power output up to 60W-100W. GreenLight HPS Laser System. KTP laser for urology procedures, 532 nm wavelength, power output up to 120W. The contractor shall provide laser operators who are certified and/or qualified, and equipment, to operate the following laser: The current scope is for the listed lasers, but potential to include other lasers as technology changes. The lasers must be kept up-to-date with manufacturer�s software updates. 102) Laser for urology procedures, 980nm wavelength, power output up to at least 180W and both pulsed and continuous treatment modes. Include disposable fibers and accessories as needed. 103) Laser for urology procedures, 532 nm wavelength, power output up to at least 120W. Include disposable fibers and accessories as needed. 104) Laser for urology procedures, 532 nm wavelength, frequency-doubled, power output up to at least 180W. Include disposable fibers and accessories as needed. 105) CO2 laser for Ear, Nose, and Throat procedures (ENT), 10600nm wavelength, continuous and pulsed treatment modes, power output up to at least 20W during continuous mode. Include disposable fibers and accessories as needed. 106) Holmium:yttrium-aluminum-garnet (Ho:YAG) laser for urology, gynecology, gastroenterology, and podiatry, 2100nm wavelength, power output up to at least 60W. Include disposable fibers and accessories as needed. 107) CO2 laser for dermatological procedures, 10600nm wavelength, power output up to at least 60 W. Include accessories as needed. 108) CO2 laser for transmyocardial revascularization procedures. Include accessories as needed. 109) Potassium-titanyl-phosphate (KTP) laser, 532 nm wavelength, power up to 40W. Include disposable fibers and accessories as needed. 110) KTP laser for ENT procedures, 532nm wavelength, power up to at least 15 W continuous and up to at least 150W during pulsed mode operation. Include disposable fibers and accessories as needed. 111) Neodymium(Nd):YAG laser for urology, gynecology, gastroenterology, and podiatry, 1064 nm wavelength. Include disposable fibers and accessories as needed. 112) Argon beam coagulator for coagulation and hemostasis of bleeding tissues. Include disposable grounding pads, laparoscopic probes, and hand pieces. 113) Frequency-doubled, Q-switch Nd:YAG laser for tattoo removal, 532nm and 1064nm wavelength. Include disposable fibers and accessories as needed. 5.1.3. The contractor shall provide the laser listed above, as required. The contractor shall set-up, break-down, and monitor the equipment and required consumables during procedures. 5.1.4. The contractor shall transport lasers and laser accessories to the MTF MOR. 5.1.5. The contractor shall provide general information to physicians and surgical staff on general laser safety and the proper operation of various medical lasers. 5.1.6. The contractor shall maintain laser equipment properly and follow all safety regulations and procedures. 5.1.7. The contractor shall provide a general report of completed services and results to the MOR Department as soon as possible after performance of services, but no later than twenty-four (24) hours after the scheduled procedure. This report shall detail any specific equipment settings that are directly related to the service provided. 6. Special Qualifications for Laser Systems Specialists: a) The medical laser systems specialists shall have completed an Associate�s Degree in Science in a Laser Technology program that included laser physics, laser safety administration, laser delivery systems, laser utilization in surgical procedures and laser treatment modalities. Training of all operators shall be equivalent to the recommendations from ANSI Z1361 �Safe Use of Lasers� and ANSI Z136.3 �Safe Use of Lasers in Health Care Facilities Standard.� b) The medical laser systems specialists shall be current with and have completed all continuing education requirements specified by their professional licensure or certification.
