36C24122Q0768 0001 Page 1 of Page 7 of 47 Page 1 of Statement of Work (SOW)  Introduction Purchase, install, and make operational (152) TVs for our elderly Community Living Center (CLC) residents in all of Bldg. 4 on our Brockton campus. 940 Belmont Street, Brockton MA 02301. The TVs must be able to be installed on existing wall mount PDI-179AV-G. Scope Install (152) TVs bedside in Bldg. 4 in Brockton. Remove and dispose of the existing (152) obsolete TVs. Programing and support for 1 year. Equipment Specifications: TVs must be 19 wifi-enabled, remote controlled, hospital-grade, smart TVs and are able to fit on existing PDI-179AV-G wall-mounts. Arm mount PDI-AA1407-GB Performance Requirements Install, configure, and test all TVs. Provide (2) 30 minute hands on training and system review with Bldg. 4 staff. (1) training to be held between 7pm 9pm. Training will include step-by-step instructions. Provide (180) laminated 1 page easy-to-read guides for TV operation. Any applicable training material will be provided to the Government in both paper and electronic copy. The Government shall retain the right to duplicate and distribute the training material for future training purposes. Contractor will remove and properly dispose of all waste/debris generated off site daily, as necessary to maintain a safe work environment throughout the performance period and at job completion. Contractor will not utilize the BHS dumpsters. All removal and disposal work will be performed in a safe and legal manner. 5 year warranty provided. Period of Performance is 30 days. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS All contractor personnel shall obtain a short-term identification badge issued by the VA Police.  Such badge shall be worn by the individual and prominently displayed at all times while on VA Property.  No employee of the contractor shall enter the project site without a valid identification badge issued by the VA.  In order to obtain a short-term identification badge, contractor personnel shall present to the VA Police a valid (non-expired) photo identification issued by a US federal, state or local government agency. Contractor personnel will be monitored at all times. Contractor personnel will follow all VA and Government rules and regulations. Deliverables Optimally operational TVs. Upon completion of the work, a review of the systems will be conducted by the Contractor with the Brockton Engineering Manager, M&O Foreman, the contract s COR, and the Brockton VA Electrician. The Contractor will provide all the manuals and operating instructions for the systems to the VA.
