Description
REQUEST FOR INFORMATION 1.0 DESCRIPTION The Veterans Health Administration (VHA) Non-Expendable Equipment National Program has identified the Powered Stand Lift Assist Without Ambulation Capability product line as a candidate for a VA-wide (otherwise referred to as national ) single Requirements contract award. The Department of Veterans Affairs (DVA) Office of Procurement, Acquisition and Logistics (OPAL) Strategic Acquisition Center (SAC) on behalf of VHA is issuing this Request for Information (RFI) in accordance with the FAR 10.002(b)(2)to determine the availability and technical capabilities of qualified sources. The SAC in conjunction with VHA is seeking a qualified source to provide Vancare, Inc. Vera-Lift V600 Battery Powered Lift Assists brand name or equal Powered Stand Lift Assist Without Ambulation Capability devices and accessories on an agency wide basis. VA intends to award a Requirements Contract with Firm-Fixed Price (FFP) orders IAW FAR 16.503, Requirements Contract to a supplier of this equipment. Vendors will be required to deliver Powered Stand Lift Assist Without Ambulation Capability to VA medical centers and facilities throughout the United States. The anticipated period of performance is for one (1) 12-month base period with four (4) twelve (12) month option periods from the date of award. However, the Government's decision as to whether or not to continue with the contract, upon annual review, will be based upon successful performance during each previous year and continued need. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 Surgical Appliance and Supplies Manufacturing and the associated size standard is 750 employees. The FAR Provision found at 52.204-7(b)(1) requires that All interested Offerors must be registered in SAM prior to submitting an offer or quotation. You may access the SAM website at https://www.sam.gov/portal/public/SAM/ . All quoters must be certified as with the appropriate NAICS code on the SAM website. All SDVOSB/VOSB offerors must be verified in the Vendor Information Pages (VIP) database at: https://www.vip.vetbiz.va.gov/ at the time of quote submission. This notice is for planning purpose only and does not constitute a commitment on the part of the Government to award a contract, nor does the Government intend to pay for any information submitted as a result of this notice. This RFI is part of a continuous process for obtaining the latest information on the commercial status of the industry with respect to their current and near-term abilities. The information provided herein is subject to change and in no way binds the Government to solicit for, or award, a competitive contract. It is emphasized that this is a notice solely for planning and informational purposes. 2.0 SCOPE Powered Stand Lift Assist Without Ambulation Capability allows a single caregiver to perform patient transfers with no manual lifting. The shin pad and leg straps are flexible and conform safely to the patient, making the sit-to-stand motion feel more natural and comfortable. They provide patients with a secure support to steady themselves during the raising and lowering transition from beds and various medical chairs. A hand controlled powered device allows the caregiver to operate the various functions of the lift assist, to include opening and closing the chassis. The low clearance chassis of a powered stand lift assist slides smoothly under most low-sitting beds and wheelchairs. First, the SC table below shall be included in the response to this sources-sought notice with a cross-reference column of the page number(s) identifying where that the requirement is met. Next, the vendor must clearly identify the item by brand name (if any) and make/model number. Finally, the vendor must include descriptive literature, such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the contracting officer, and clearly describe any modifications it plans to make to a product to make it conform to the solicitation requirements. The Department of Veterans Affairs (VA) is seeking vendors who can provide Vancare, Inc. Vera-Lift V600 Battery Powered Lift Assists Without Ambulation Capability equipment and accessories as listed above or equal commodities which meet all of the following salient characteristics: CLIN 0001 apply to the following SCs: SC # SALIENT CHARACTERISTICS METHOD OF EVALUATION Document Title and Page # 1 Must have capability to transfer partially weight bearing patients who participate in the raising motion to and from a chair, wheelchair, toilet, or bed. Literature Review 2 Must have a shin pad that is adjustable or flexible to avoid pressure on the knee. Literature Review 3 Must have or offer a leg strap to secure legs to shin pad. Literature Review 4 Must have emergency stop and emergency lowering without power. Literature Review 5 Must have at least 4 casters that swivel. Literature Review 6 The rear casters must have brakes. Literature Review 7 All permanently attached parts of the lift, including staff and patient grasp bars, controls, and leg straps, must be able to be disinfected. Literature Review 8 Warranty must be at least 1 year. Literature Review 9 Expected equipment life must be at least 8 years. Literature Review 10 Casters under the bed must have an option with leg clearance of no higher than 4 ΒΌ inches. Literature Review 11 Lift must have battery status indicator. Literature Review 12 Foot plate must not be removable Literature Review CLIN 0002 apply to the following SC: SC # SALIENT CHARACTERISTICS METHOD OF EVALUATION Document Title and Page # 13 One charging option must be a removable battery and a wall mounted charger. A direct plug to the wall may also be offered as an option. Literature Review CLIN 0003 0006 apply to the following SCs: SC # SALIENT CHARACTERISTICS METHOD OF EVALUATION Document Title and Page # 14 Lift and any slings, harnesses, or straps must have a weight capacity 440 pounds. Literature Review 15 Must be able to provide disposable and/or wipeable slings in a variety of sizes from small up to the full capacity of the lift or be able to provide a written statement allowing using other manufacturers standing slings. Literature Review The Government's intent is to include all ancillary items to ensure the equipment can function as designed by the OEM and as clinically required. Vendors are encouraged to provide any product solution or configuration so long as they meet the salient characteristics. Additionally, responses can include any additional product(s) that may be beneficial. These items must be clearly identified by brand name and part number. Responses to this Sources Sought Notice shall include the following: 1) Full name, address, and phone number 2) DUNS/ CAGE/ SAM UEI 3) Business Size 4) Original Equipment Manufacturer name and address 5 ) Ability to provide uninterrupted supply of products on a national scale (CONUS) 6) Technical Literature that clearly shows product(s) meet the identified salient characteristics (if submitting an or-equal item)
