- The Lubbock Outpatient Clinic Laboratory, part of the Amarillo VA Health Care System (AVAHCS), is seeking Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), or Small Business sources capable of meeting the requirement listed below. The acquisition will be accomplished using commercial item procedures in accordance with FAR Part 12. NOTE: TO QUALIFY SDVOSB AND VOSB VENDORS MUST BE CERTIFIED IN THE VA VENDOR INFORMATION PAGES (VIP) DATABASE. The North American Industry Classification System (NAICS) is 325413. Any SDVOSB, VOSB, or Small Business firms who wish to identify their interests and capability to provide this product must provide product specifications, performance, and delivery information by notifying the Contract Specialist no later than 4:00 PM Central Time, October 10, 2022. Notification shall be e-mailed to Robyn Nussbaum, at robyn.nussbaum@va.gov. Phone calls will not be accepted. Any vendor who responds to this Notice must provide credentials to perform the requirement as outlined in the below scope. All information received in response to this Notice that is marked as proprietary will be handled accordingly. In accordance with the Federal Acquisition Regulation, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Notice. SCOPE The Lubbock Outpatient Clinic Laboratory, part of the Amarillo VA Health Care System (AVAHCS), Pathology and Laboratory Medicine Services (P&LMS) is looking for a straight cost contract for supplies, consumables, controls, and calibrators. The reagents and consumables must be compatible with the Radiometer ABL 90 Flex analyzer that is currently in place. No other types of substitute or third-party reagents and consumables will fit the need of the Lubbock Outpatient Clinic Laboratory for the reasons noted. In addition to being the best value to meet the needs of P&LMS for the processing/analyzing in-house arterial blood gas assays to either replace or to sustain, without incurring testing delays. The current testing analyzers are in place located at: Lubbock Outpatient Laboratory 3510 4th Street Lubbock, TX 79415 Attention: Laboratory GENERAL REQUIREMENTS The Vendor shall provide, in accordance with all applicable federal, state, and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, supplies, and services as negotiated on GSA contracts to enable AVAHCS to analyze patient specimens accurately and efficiently. Supplies, consumables, controls, and calibrators will consist of cost per test request for a base year and up to four (4) possible option years to be exercised at the government s discretion. The Vendor shall provide installation support services, supplies/consumables, and training (on and off site) to include the performance of analytical testing as defined by the vendors published FDA approved parameters. All requirements and provisions defined in the specification of this solicitation will apply to any vendor, i.e., branch, division, sub-Vendor, etc. supplying parts or reagents on behalf of the vendor. For ABL5/5XX, ABL7XX, EMIL 10X Unknown 2 per year VENDOR RESPONSIBILITIES The vendor shall provide all supplies, consumables, controls, and calibrators suitable to provide the Lubbock Outpatient Laboratory with the ability to analyze patient arterial blood gas specimens accurately and efficiently and must be capable of performing the following tests. The vendor shall list the reagents, controls, and calibrators that require any additional preparation prior to use such as warming/mixing/reconstitution. The vendor shall list the reagents/waste/receptacles requiring treatment before disposal. The vendor shall also list all reagents/waste that is considered above normal toxicity and/or hazard. CONSUMABLE SUPPLIES: Consumable supplies will be straight cost for this procurement with the ability to sequester lot numbers in abundant supply. Consumable supplies needed to produce the estimated annual volume (see 1. General Requirements) and shall include the following: Printer paper, 60 rolls total per year (or equivalent to process estimated annual testing volume) for both analyzers. Linearity Verification kit 20-25 vials total per year (or equivalent to process estimated annual testing volume). Reagent Pack/Solution Pack/Cartridge Pack, 26-30 packs total per year (or equivalent to process estimated annual testing volume) per location site. Hemoglobin Calibration (if required), 4-6 vials total per year (or equivalent to process estimated annual testing volume) per location. Tri-Level (H, N, L) External QC material to be available for any reagent change and/or follow up with troubleshooting by staff or by service representative for QC failures. The quantity required is the equivalent to the process of the estimated maximum annual testing. The majority of the supplies are to be stored at room temperature (due to constraints of refrigeration space at the customer s facility). Once testing is complete, the instrument sends the result back to the SNI which then sends the result back to VISTA in the appropriate format. OTHER: The vendor shall reimburse the facility for any costs/supplies related to tests sent to a referral laboratory when testing is not available through no fault of the facility due to the reagent problems, or other product issues. The assessed damages shall be applied to the invoice in the form of a credit or deduction. In the event that the any consumable, reagent, or other necessary items are found to be defective and unsuitable for use with the vendor s equipment, or the Vendor has failed to comply with the requirements for routine supply delivery, the Vendor shall deliver the consumable supplies within a period of 24 hours after receipt of the verbal order for priority delivery from the facility. If either circumstance has occurred, the Vendor shall deliver to the facility, in the most expeditious manner possible, without additional cost to the facility, the necessary consumables in sufficient quantity so testing is not delayed. This contact should assist the Lubbock Outpatient Clinic to pursue the goal of being a good steward in our community by eliminating, minimizing, or mitigating adverse environmental impacts. The vendor shall assure its processes do not create an unclean and unsafe environment for the Lubbock Outpatient Clinic and do not damage or destroy government property. RECORDS MANAGEMENT AND LANGUAGE FOR CONTRACTS: Citations to pertinent laws, codes, and regulations such as 44 U.S.C chapters 21, 29, 31, and 33; Freedom of Information Act (5 U.S.C 552a); 36 CFR Part 1222 and Part 1228. Vendor shall treat all deliverables under the contract as the property of the U.S Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determined to be in the public interest. Vendor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and / or Government records. Vendor shall not retain, use, sell or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Vendor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data / records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Vendor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Vendor agrees to comply with Federal; and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Vendor is required to obtain the Contracting Officer s approval prior to engaging in any contractual relationship (sub-Vendor) in support of this contract requiring the disclosure of information, documentary material and / or records generated under, or relating to, this contract. The Vendor (and any sub-Vendor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. LICENSING/ACCREDITATION SECURITY/CONFIDENTIALITY: The vendor shall have all licenses, permits, accreditation and certificates required by law and or regulatory compliance. Personnel assigned by the vendor to perform these services covered under this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The C&A requirements do not apply, and that a Security, Accreditation Package is not required.
