- This is a 100% SDVOSB Set-Aside Project Number: 436-23-109 Project Name: X-Ray Lab Room 121 Floor Replacement Purpose: The current elevated floor in room 121 is a safety hazard that needs to be removed completely and replaced with Luxury Vinyl Tile (LVT) flooring. Existing Site Conditions: The existing elevated floor was originally installed to provide access to electronic equipment supporting the X-Ray machine (e.g., cables, connections, etc.). However, due to equipment modernization efforts, there is no longer use/need for this floor and it must be removed and the floor underneath tiled to provide a better environment to the occupants. (See photos 1-4 of attachment 2) It will be the responsibility of the Contractor to perform site investigation to identify site conditions prior to submitting cost proposal. General Contractor Requirements: Conform to ICRA requirements: Class II/III Room 121 (Office & Closet): Work in Room 121 involves removing the elevated floor, baseboard, casework, old finishes (ceiling tiles, trim, etc.), and updating w/ new flooring & casework. The existing tile underneath the elevated flooring is known to have asbestos containing materials. The asbestos will be abated if necessary but the intent is to place the LVT material on top of the existing tiles if possible. Wall and floor finishes will be updated to align with the recommended standards in the Design Guide, walls will meet a level 4 finish and the floors will be a LVT that meets the approved VA finish schedule (provided upon request). The pattern will be selected by the VA Interior Designer and/or COR through the submittal process. The existing electrical and data cables will be routed overhead from the room to the machine room. Currently, these cables are routed through a floor conduit that will be covered (patched) under the new flooring. (See photo 5 of attachment 2) The contractor will be responsible for removing the existing furniture and returning it once the project is completed. Period of Performance: The expected period of performance is 90 days from Notice to Proceed. Work Schedule: Work to be completed after normal hours of operation to minimize disruption to clinical activities and equipment downtime. This will require extensive coordination w/ the staff. Information Security: The C&A requirements do not apply, a Security Accreditation Package is not required. Incidental exposure to protected health information may occur from accessing the work site. Staff will follow all Privacy & Security policies and procedures.
