- 10/13/22:� The answer the the questions are being provided to the offerors.� �A total of 121 questions were received.� The following documents were udated as a result of the questions and updated versions are being provided.� The following documents are attached: Question and Answers Template (121 questions / answers) Revised Solicitation - see v3 dated 10/13/22 Revised PWS - see v2 dated 10/13/22 Revised Pricing Template - see v2 dated 10/13/22 Revised Contractor Assurance� - see v2 dated 10/13/22 Revised Past Performance Questionnaire - see v2 dated 10/13/22 (note that this form shall be returned from your Client directly to sharon.edwards3@fema.dhs.gov and janice.joseph@fema.dhs.gov via email only.� The language in the first paragraph stating that the questionnare shall be returned to the offeror by the due date has been deleted.�� NOTE:� �For changes in the Solicitation and PWS, the Contracting Officer has attempted to highlight in green all changes, however it is the offerors responsibility to scan the initial and revised documents to ensure all changes are captured. The due date for Phase I proposals is still, Monday, October 17, 2022 at 5:00 pm EST 10/12/22:� � The phase I proposal due date is being changed from Friday, October 14, 2022 at 2:00 pm to Monday, October 17, 2022 at 5:00 pm EST.� �The answers to the quesstions and a revised solicitation will be posted later today, October 12, 2022 or first thing in the morning, October 13, 2022.�� 10/04/22:� � � A revised solicitation is attached,�see,� 2.2.1 SOLICITATION 70FA4023R00000001 - Schedule thru Section M (v2 200422).�The questions and proposal due dates were corrected on page 2 to match the dates listed in Section L.3 (Proposal Preparation Instructions and Content). Title:� FEMA Medical and Behavioral Health Services The Federal Emergency Management Agency (FEMA) carries the vital responsibility of leading the effort to secure our Nation. The FEMA workforce of skilled and dedicated employees is the foundation on which� its ability to protect our nation is built. The Office of the Chief Administrative Officer (OCAO) is responsible for Workforce Health and Safety for the FEMA.� Currently OCAO has a requirement to provide medical and behavioral health services to the FEMA workforce. The services required under this contract are divided into two operations:� Fixed Sites (Non -Disaster sites) and Disaster sites.� Each operation is independent of each other; however, services may be similar at both locations and may be requested at the task order level.� These services include but are not limited to the following:� 1.�������� Medical and Behavioral Health Oversight 2.�������� Occupational Health Center Services and clinical staffing support 3.�������� Extended nursing hours to support the National Recovery Communication Center 4.�������� (NRCC) and/or Regional Recovery Communication Center (RRCC) 5.�������� Medical Employability, Fitness for Duty, Consultation 6.�������� Tele-Medical/Health Services 7.�������� Special Testing, COVID-19 � � � � � �a.� Mobile Application for proctored testing 8.�������� Immunizations and travel medication 9.�������� Respiratory clearances 10.������ Commercial driver�s license (CDL) clearances 11.������ Optional: Employee Assistance Program (EAP)/Enhanced Work Life 12.������ Behavioral Health Stress Counselors Important Due Dates:� Questions due Date:� Monday, October 10, 2022 at 2:00 pm (EST) (Questions for both Phase I and Phase II are due by the Questions due date).� Offerors shall use the Questions Template, (see Attachment 6, Questions Template)� Form shall be submitted in Excel Format. Phase I Proposal Due Date:� Friday, October 14, 2022 at 2:00 pm (EST) :� Page Limit:� Written submission totaling no more than 7 pages.� Additional Pages:� the only other pages that are allowed are a 1-page cover sheet, a 1-page table of contents and a 1-page Executive Summary which shall only restate the contents of the proposal.� The Phase I proposal shall be submitted in MS Word or PDF Format. Phase II Proposal Due Date: The due date for Phase II proposals is Friday, October 28, 2022 at 2:00 pm (EST).�� Page Limit:� The Technical Proposal submission shall total no more than 15 pages (excluding cover sheet and resumes).� Additional Pages: the only other pages that are allowed are a 1-page cover sheet, a 1-page table of contents and a 1-page Executive Summary which shall only restate the contents of the proposal
