Description
**Amendment 2 - Adding additional requirement for Jordanian citizens: 10. Copy of official criminal record history (required even if there are no incidents on file). Also underlined in requirement bellow. ** **Amendment 1 - This amendment is to provide interested parties with a new DMS application. Please use the form labled ""DMS Application - New"" when submitting your request for access to the site visit.** It is anticipated that a construction site-visit will be conducted in late January 2023. Personnel who are interested in participating are required to submit an application package to USACE for processing. No more than two personnel from each company are allowed to participate in the site visit. However, USACE reserves the right to limit the number of personnel to one (1) company representative should circumstance require and we will inform all companies at least 24 hours in advance of the meeting should this become necessary. All documents must be typed � No handwritten paperwork will be accepted. All packages must include the followings forms: Jordanian Citizens�must submit the following forms (electronic and hard copies) to USACE: Two (2) 5�x5� color passport photos Four (4) copies of DMS Application Copy of National ID, front and back Copy of Family Book DMS Clearance Worksheet (Included as an attachment to this post) - Pass Start: 23 January 2023 / Pass End: 28 February 2023 332 AEW Vetting Request Worksheet.xlsx (Included as an attachment to this post) - Pass Start: 23 January 2023 / Pass End: 28 February 2023 Company Details (Who you are, What you do, Where you are located) Owner�s full information to include contact phone number Copy of Government Licensing Copy of official criminal record history (required even if there are no incidents on file) Personnel who are from�other countries and are not Jordanian citizens, including U.S. Citizens, must submit the following forms (electronic and hard copies) to USACE: Two (2) 5�x5� color passport photos Four (4) copies of DMS Application Copy of Passport DMS Clearance Worksheet (Included as an attachment to this post) - Pass Start: 23 January 2023 / Pass End: 28 February 2023 332 AEW Vetting Request Worksheet.xlsx (Included as an attachment to this post) - Pass Start: 23 January 2023 / Pass End: 28 February 2023 Company Details (Who you are, What you do, Where you are located) Owner�s full information to include contact phone number Copy of Government Licensing The application packages and any questions related to this topic shall be submitted via email to both of the USACE point of contacts identified below no later than 10:00AM local Jordanian time, 14 November 2022: MAJ Robert M. Fullerton at robert.m.fullerton@usace.army.mil �- Schedule delivery of hard copies for the required information to MAJ Fullerton. Weekly appointments are available every Thursday in Amman, Jordan. Mr. Steven J. Ziemba at steven.j.ziemba@usace.army.mil Be advised, that even if the application package is submitted consistent with these instructions, approval to access the site for the organized site visit is not guaranteed by submission. Companies that successfully prequalify for base access will be provided additional information regarding date, time, and arrival location. �Companies that fail to qualify for base access will not be allowed to participate in the site-visit. Attendees will be escorted to the project site but should allow time for security screening at the entrance of the base. Photography while on base will not be permitted. All company representatives will be required to stay in one group at all times and be escorted by the USACE representative at all times.� At no time will company representatives be allowed to leave the site visit group or walk anywhere on base without a USACE representative. Note: Interested contractors do not have to attend the site visit to submit proposals for this procurement.
