All information received in response to this RFI that is marked Proprietary will be handled accordingly. Responses to the Sources Sought will not be returned. At this time, questions concerning the composition and requirements for a future Request for Quote will not be entertained. The Department of Veterans Affairs is conducting market research in order to provide Laboratory Analysis Support for Industrial Hygiene, Environmental and Water Quality Samples at the Martinsburg WV, VA Medical Center. The Government anticipates awarding a Firm Fixed Price IDIQ type contract that will provide the support services listed within the below Draft Statement of Work. OBJECTIVE: See Draft SOW. ATTACHMENT 2 PRICING SCHEDULE 9 STATEMENT OF WORK The Martinsburg VA Medical Center (MWV) located in Martinsburg, WV operates a medical hospital, rehabilitation, and long term care units on a campus of approximately 180 acres. Inherent with the operations of the hospital is the potential to affect the safety, health, and well-being of staff, patients, and the public as a result or workplace exposures typically found in a healthcare setting. MWV also includes seven Community Based Outpatient Clinics (CBOCs) in Maryland, West Virginia and Virginia. The Contractor shall provide full-service industrial hygiene, environmental and water quality analyses for samples collected at the MWV or its CBOCs on a cost per sample basis. Samples will be collected by MWV and hand delivered or mailed to the Contractor for analysis and reporting. The contractor must have the capability to provide reports electronically. The contractor shall provide sample media for typical environmental samples, e.g., MCE filter cassettes, mold/mild lift samples, swabs, XAD tubes, treated water sample bottles, coolers, etc. A list of requested testing procedure is listed in attachment 2. On occasion, additional testing services may be requested or required; therefore, the contractor must be a full-service industrial hygiene, environmental and water quality laboratory. These analyses should be accomplished by an experienced staff with diverse analytical expertise; with state-of-the-art analytical equipment; using OSHA/NIOSH/EPA, or equivalent methods, and approved West Virginia requirements and analytical techniques; offer priority turn-around times on sample processing and method and results consultations. QUALIFICATIONS Company Experience, Licensure and Accreditations Company must have experience and certification to provide industrial hygiene, environmental and water quality laboratory analysis /services. The contractor must be able to meet specific hold time criteria. For some of the analyses requested, this requires a laboratory facility within no more than 2 hours from the main medical center campus. The laboratory shall also be capable of providing requested turnaround times for results. Company has no notices of violations on file with OSHA/EPA/WVDEP for at least the last 5 years. Laboratory shall have the following current accreditations: AIHA/EMLAP/IHLAP/ELLAP AIHA/EMPAT/IHPAT/ELPAT NVLAP CDC Elite Legionella Certificate of Proficiency Laboratory shall have the following licensure from West Virginia (WV) Department of Health and Human Resources (DHHR): Asbestos Air and Bulk Sampling Analysis Radon Laboratory shall also be accredited under the WV Department of the Environment (DEP) Laboratory Quality Assurance Program for the following: Non-Potable Water Solid & Chemical Inorganic Nonmetals Volatile Organic Chemicals Trace Metals Whole Effluent Toxicity Hazardous Waste Characteristics Extractable & Semi-Volatile Organic Chemicals Quality Control/Quality Assurance Program Contractor shall have a written quality control/quality assurance (QA/QC) program for sample analysis, data assessment and report review and submissions. Subcontractors Any and all expenses incurred, should sub-contracting be authorized, will be borne by the Contractor (prime) without premium to the government. The prime contractor must self-perform 51% of the work of the contract. Contactors shall submit qualifications for sub-contractors at the time of bid. Subcontracting work to vendors not submitted during bid is forbidden without the express, written consent of the COR. PROCEDURES/GENERAL REQUIREMENTS Contracting Officer Representative The Contracting Officer Representative (COR) for this contract is: Andrew Faust Industrial Hygienist Safety Office Martinsburg VA Medical Center 510 Butler Avenue Martinsburg, WV 25405 Phone: 304-263-0811 x2567 FAX: 304-262-1401 E-mail: Andrew.Faust@va.gov The COR monitors all technical aspects of the contract and assists in contract administration and is authorized to perform the following functions: Ensure that the Contractor meets the technical requirements of the contract Perform inspections necessary in connection with the contract performance Maintain written and oral communications with the Contractor concerning technical aspects of the contract Monitor Contractor s performance and notify both the Contracting Officer and Contractor of any deficiencies Review and approve payments of services completed by the Contractor A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates, or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order. General applicability of codes, regulations, and standards All work under this contract shall be done in strict accordance with all applicable federal, state, and local regulations and VA polices. All applicable codes, regulations and standards are adopted into these documents and will have the same force and effect as these contract documents. The most recent edition of any relevant regulation, standard, document or code shall be in effect. Where conflict among the requirements or with these contract documents exists, the most stringent requirement(s) shall be utilized. PRESENTATION OF TASKS FOR BILLING/INVOICING Contractor will bill per chain of custody for submitted samples. Invoices shall include the analysis method, number of samples, cost per test, project title/name, and purchase order for the current billing cycle. SUBMITTALS REQUIRED FOR BID Contractor shall provide information as identified within Attachment 1 Evaluation Factors for Award. PRICE/COST SCHEDULE Contract is for Base Year and 4 Option Years. This is an estimate based on cost per test services and estimated usage per year. All invoices submitted will be verified with the cost per test in accordance with price schedule provided in the RFP. Laboratory to provide per test pricing for the services requested in Attachment 2. Costs shall INCLUDE the analysis and ALL required media, e.g., sorbent tubes, passive badges, and/or return shipment containers, e.g., boxes, cooler inserts, cold packs, required for the method. Cost to ship media to MWV shall also be included in the cost per test. Unless otherwise specified by offeror, MWV will incur sample shipment costs. ATTACHMENT 1 EVALUATION FACTORS FOR AWARD Basis for Contract Award The Government will issue a purchase order to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Evaluation Approach The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The evaluation will consider the following: Technical or Quality: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government s requirements as outlined in the solicitation and based on the information requested in the instructions to quoters section of the solicitation. Past Performance: The past performance evaluation will assess the relative risks associated with a quoter s likelihood of success in fulfilling the solicitation s requirements as indicated by the quoter s record of past performance. The past performance evaluation may be based on the contracting officer s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis. Price: The Government will evaluate the price by adding the total of all line item prices. The Total Evaluated Price will be the sum of all line item prices. Technical Factors The following technical factors shall be used to evaluate offers as part of the comparative evaluation: Experience Technical Capability Management Approach Procedures for Services requiring a Statement of work. This process permits comparative evaluation among price and non-price factors and allows the Government to accept other than the lowest priced offer. Evaluation technical factors are listed in decreasing order of importance. All technical factors when combined are slightly higher than price. Comparative evaluations will be used to review information submitted on the following factors: Experience Information should be submitted that will allow the Government to have confidence in the respondent s ability to successfully perform on the contract. The contractor should show a successful past experience with same or similar requirements, laws, licenses, and certifications. Offeror must demonstrate within their response previous laboratory work experience by submitting information which includes the type/number of work projects similar to extent of work of this contract conducted within the past 5 years, as well as specific information on the type and size of facilities where work was performed, and the services provided. The offeror will provide a minimum of 3 past performance references for work similar in cost and technical scope. Past performance references shall identify the Agency name and the point of contact phone number, contract or delivery order number, a brief scope of contract, and contract value. Past performance must be within the last 5 years. Provide similar past work experience for any subcontractors proposed on this contract. Their experience provided can be tailored to the specific sector(s) of the scope of work to which they will provide services. Technical Capabilities The contractor shall show the necessary expertise, certifications, security/compliance, insurance, and licenses to accomplish the work described in the Statement of Work, Scope and Requirements. The contractor shall also have the technical capability to meet all performance levels as stated in the Scope and Requirements section of the RFQ. The Contractor must possess the following: Certification in the following laboratory accreditations: AIHA/EMLAP/IHLAP/ELLAP AIHA/EMPAT/IHPAT/ELPAT NVLAP CDC Elite Legionella Certificate of Proficiency Licensure from WV DHHR in the following analysis methods: Asbestos Air and Bulk Sampling Analysis Radon Laboratory shall also be accredited under the WV DEP Laboratory Quality Assurance Program for the following: Non-Potable Water Solid & Chemical Inorganic Nonmetals Volatile Organic Chemicals Trace Metals Whole Effluent Toxicity Hazardous Waste Characteristics Extractable & Semi-Volatile Organic Chemicals Management Approach / Quality Assurance Offeror shall propose its method of response, within a Standard Operating Procedures Plan for accomplishing levels of performance as stated in the Statement of Work. Offeror shall also propose a plan detailing its Quality Assurance Control and Surveillance plan and a proposed plan to meet all items listed in the Statement of Work. Any subcontracting plan or teaming agreements established to perform the requirements of this solicitation must show evidence of all needed licensing, permits, and certifications pertaining to their responsibilities as identified within. The Prime offeror must submit evidence that the subcontractor is qualified and legally able to provide services. Prime offeror shall submit resumes of any key personnel along with references for subcontractors in the same manner as required for the prime offeror. The offeror should develop a plan showing an understanding of the tasks to be covered and the schedule for the required services. The plan shall, at a minimum: Provide a management organization plan that, among other items, shows how the work will be managed to ensure the success of the contract from a professional, cost, and time perspective. Provide a plan that ensures adherence to hold times and turnaround times required. Past Performance The past performance evaluation will assess the relative risks associated with a quoter s likelihood of success in fulfilling the solicitation s requirements as indicated by the quoter s record of past performance. The past performance evaluation may be based on the contracting officer s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis The offeror will provide a minimum of 3 past performance references for work similar in cost and technical scope. Past performance references shall identify the Agency name and the point of contact phone number, contract or delivery order number, a brief scope of contract, and contract value. Past performance must be within the last 5 years. Firms with past performance experience in health care field, industrial operations and construction/renovation who can also illustrate VA experience will be preferred. Provide similar past work experience for any subcontractors proposed on this contract. Their experience provided can be tailored to the specific sector(s) of the scope of work to which they will provide services. Cost Evaluation Criteria If two or more offers are considered technically equivalent, price or cost may become of primary importance in determining the proposal most advantageous to the Government. The Government will evaluate the price by adding the total of all line item prices. The Total Evaluated Price will be that sum. All PROPOSALS SUBMITTED WITHOUT ALL REQUESTED INFORMATION AND DOCUMENTATION WILL BE DEEMED NONRESPONSIVE. ATTACHMENT 2 PRICING SCHEDULE It should be noted that the testing listed below includes typical testing conducted. On occasion additional testing may be necessary; therefore, the Contractor must be a full service Industrial Hygiene, Environmental and Water Quality Laboratory. Provide costs per test associated with each turnaround time. If the listed turnaround time is not available due to methods, mark the block with N/A . If the Contractor does not provide the requested analysis, mark the block with N/P . Contractor shall note any testing which will require the use of a subcontractor.
