This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ), and the solicitation number is 36C24423Q0096. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-08 effective October 28, 2022. The Department of Veterans Affairs Altoona VA Medical Center, 2907 Pleasant Valley Blvd, Altoona, PA 16602-5610 is seeking to procure the PM Certification and CM of Audiology Equipment. All interested companies shall provide quotations for the following: The anticipated Period of Performance will begin on 11-11-2022 following Contract award: See also Attachment 1, Price Cost Schedule. Place of Performance Address: Department of Veterans Affairs VAMC Facility as well as its five (5) Community Based Outpatient Centers (CBOCs): Altoona VAMC main site James E. Van Zandt VA Medical Center 2907 Pleasant Valley Blvd Altoona, PA 16602 Johnstown VA Clinic 598 Galleria Drive Johnstown, PA 15904-8900 State College VA Clinic 2581 Clyde Avenue State College, PA 16801-7508 DuBois VA Clinic 5690 Shaffer Road DuBois, PA 15801-3870 Indiana County VA Clinic 1570 Oakland Avenue Regency Mall Suite 100 Indiana, PA 15701-2429 Huntingdon County VA Clinic 13903 William Penn Highway Mapleton Depot, PA 17052-9649 All work shall be completed in accordance with the Statement of Work (SOW). The SOW is included as Attachment 1 to this solicitation. Offerors are requested to quote Firm Fixed Price for any and all of the line items on the Schedule of Supplies and Services attached to this solicitation. The Schedule of Supplies and Services is included as Attachment 1 to this solicitation. Service Contract Act Wage Determination Number 2015-4221 Rev 21 dated 05-27-2022, applies to this acquisition and will be incorporated into the resulting contract. A copy of the Wage Determination is included with this solicitation as Attachment 2. Any award resulting from this solicitation will be issued on a Standard Form 1449. NOTE: To receive an award resulting from this solicitation, offerors MUST be registered in the System for Award Management (SAM) database IAW FAR 52.212-(k).
