STATEMENT OF WORK (SOW) FOR VISN 15 Ancillary Testing: COVID 196 Antigen Testing INTENT: It is the intent of the Department of Veterans Affairs, (herein afterwards referred to as VISN 15) to establish an agreement for Point of Care Reagents, Test Kits & Sets for COVID-19 Antigen test and controls. Contractor agrees to the following terms with the VISN facilities listed by Attachment A herein and awarded. However, as requirements change, facilities within VISN 15 may be added or deleted by supplemental agreement of the Government and the Contractor. ORDERS: The Government is obligated only to the extent of authorized orders issued under the agreement by authorized individuals. PRICES AND TERMS: VISN 15 will provide an estimated volume by test as reflected in Attachment B for each individual medical center and outpatient clinic laboratory. Pricing is based on the AVERAGE daily test volume for each facility. The Government estimates the volumes per facility as listed in Attachment B but does not guarantee volumes as listed; they are estimates ONLY. TERM OF agreement: This will be a single vendor award, firm-fixed price agreement with one base year and three, one-year options. This agreement is not a contract. VISN 15 intends to establish the base year of the agreement for the period of 12/01/2022 through 11/30/2026. IDENTIFICATION: Delivery Orders issued shall be identified by their applicable agreement Number. Agreement identification numbers are assigned through the VHA. ORDERING METHOD: The participating facilities in Attachment A may order products via Electronic Data Interchange (EDI), telephone, facsimile, or other written communication, identifying the products by number, quantity, purchase price, address for delivery, and any special instructions. DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK SCOPE OF PROCUREMENT: Contractor shall provide the following test kits and controls required to perform rapid COVID-19 screening to patients and employees according to the delivery schedule. GENERAL REQUIREMENTS Technical Features Test kits must be Clinical Laboratory Improvement Amendments (CLIA) waived. The COVID-19 Ag swab test must be acceptable to use with individuals suspected of COVID-19 in all sections of the hospital, including patients with bleeding disorders, spinal cord injury, traumatic brain injury, and patients undergoing major surgery for head and neck diseases. Testing time should be equal or less than 15 minutes. Test procedure must include minimal pre-treatment steps post collection. Test procedure must include minimal sample manipulation to decrease potentially infectious exposures. Test Kits from the same shipment must have the same lot numbers and expiration date. Packaging and Inspection: All kits will be packaged with adequate materials to ensure safe handling. If a kit is delivered and upon inspection the materials are found to be defected, the contractor shall deliver a new kit immediately upon notification at no additional costs. Quality QC material should be included in every box to meet regulatory standards. QC material should at minimal include a positive control. Additional shipments of the same lot number must include additional QC material. If 3-5% of the kits fail to perform, it will be reported to the contractor and the contractor shall send a replacement test kit at no additional charge. Attachment A: Harry S.
