Description
This requirement is seeking potential vendors of�the Eyelation Kiosk with access to the full catalog of safety glasses and prescription lenses offered by Eyelation.�The Eyelation Self Service Kiosk uses the camera of a tablet to upload individual prescriptions and utilizes proprietary and patented software to measure the employee's pupillary distance.�� The provided prescription safety glasses frames and lenses shall meet or exceed American National Standards Institute (ANSI) Z87+ as required by Occupational Safety and Health Administration (OSHA) 1910.133 regulations.� Provided frames and lenses shall also comply with Trade Agreement Act requirements. See the attached Product Description for more details. REQUIRED SUBMISSIONS All vendors shall submit the following: 1) A Firm Fixed Price (FFP) quotation 2) Cage Code, DUNS Number, delivery timeframe after receipt of order for glasses 3) This is a Solicitation for a five (5) year�Blanket Purchase Agreement (BPA). The Government intends to award the BPA as a result of this Solicitation that will include the terms and conditions that are attached. In order to facilitate the award process, ALL quotes shall include a statement regarding the terms and conditions attached as follows: The offeror shall state ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR The offeror shall state ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Offeror shall list exception(s) and rationale for the exception(s). Evaluation Criteria Section L & M: Evaluations will be conducted per FAR 52.212-2, Evaluation -- Commercial Items (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) price; (ii) technically acceptability as specified in the attached purchase specification (b) Evaluation is on Lowest Price Technically Acceptable (LPTA) basis. All offers will be evaluated on their proposed Total Evaluated Price (TEP). The contract will be awarded to the company with lowest price offers, which also fully meets all specifications. Only the lowest priced offer will be evaluated for technical acceptability. The next lowest offer will only be evaluated if one of the aforementioned offers is not found technically acceptable (and so on). Award will be conducted under the provisions of FAR Part 12, Commercial Items, and FAR 13, Simplified Acquisition Procedures. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding agreement without further action by either party. Before the offer�s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
