Description
Project # 623-23-107 Renovate for Audiology Space Request contractor provide all required labor, material, equipment, and site investigations to complete renovation of the space as described below. The objective of this project is to remodel audiology space on 5th floor B-wing of building 1 (refer to attached pdf drawing for reference). The entirety of space is approximately 922 sq. feet. Renovation area include room 5B35-1, 5B38-1, 5B39-1, 5B40-1, 5B41-1, 5B47-1, 5B49-1, 5B50-1, and 5B51-1. The remodeled space will require new luxury vinyl tile flooring, wall repair and paint, tile and wall demo and the addition of a new mop sink, and three wall hung sinks. During construction, remove all furniture and relocate to a secure storage area. All spaces shall be inspected for asbestos and lead paint prior to demolition work. Prices should include the abatement of lead paint and asbestos mastic beneath existing floor tile. The remodel should include the following: Room Addition Demolish 5B35-1 New flooring/wall repair and paint. New wall mount sink. Demo VCT flooring including mastic abatement 5B38-1 New flooring/wall repair and paint Demo ceramic tile flooring and wall. Demo wall sink. 5B50-1 New flooring/wall repair and paint Demo VCT flooring including mastic abatement 5B51-1 Add mop sink and repair and paint upper wall 5B39-1 New flooring/wall repair and paint. Combine space with 5B-40. New wall mount sink. Demo tile showers and tile from walls and floor. Demo wall between 5B-39 and 5B-40. 5B40-1 New flooring/wall repair and paint. Replace door entry with wall. Repair Ceiling in area of wall removal to provide continuous ceiling through area. Demo VCT flooring including mastic abatement. Demo and remove door and frame to 5B-40. 5B41-1 New flooring/wall repair and paint. New wall mount sink. Demo VCT flooring including mastic abatement 5B47-1 New flooring/wall repair and paint Demo VCT flooring including mastic abatement 5B49-1 New flooring/wall repair and paint Demo VCT flooring including mastic Renovation considerations shall include but are not limited to the following: A phasing plan shall be developed to mitigate disturbance to the department. The space must remain fully functional during construction. Existing wall art painted directly on the walls shall remain untouched. All removable wall art shall be removed during construction, stored in a safe location, and returned to the same location upon completion. Flooring to be Milliken LVT Wood #EUC 122 Eucalyptus Salinga Planks 7 x48 . Wall paint to be Sherwin Williams #SW-7649 Silverplate Satin Enamel, cove base to be Roppe Pinnacle color: Charcoal 123 Standard Toe 4 . Contractor to meet all VA Infection Control regulations while working in the area, including but not limited to ICRA barriers, negative air and walk off mats. Prime contractor shall comply with NFPA, NEC and Life Safety Codes, ADA, VA Design Criteria / manuals, construction standards and other required standards for the development of construction documents. Prime contractor shall provide one year of maintenance and inspection service following beneficial use. The space conditions and the execution of this project shall proceed with minor interference to the operation of the medical center and shall comply with all requirements for infection control, utility interruptions, fire protection, and all other safety requirements of the contract. Construction is to be completed in 180 calendar days from notice to proceed.
