Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, and FAR part 13.106-1(b)(1), Soliciting from a single source.� The Defense Health Agency (DHA) intends to negotiate on a sole source basis in accordance with FAR part 13.106-1(b)(1).� The proposed source is Doximity, Inc., 500 3rd Street, Suite 510, San Francisco, CA 94107-6803.� This acquisition is for the procurement of twenty-nine (29) AMiON Enterprise Software Licenses, including all-inclusive labor, equipment, tools, parts, materials, and preventative maintenance (PM) and corrective maintenance (CM) support necessary to ensure proper functionality of the AMiON enterprise software and/or hardware utilized by the 633rd Medical Group (MDG), Joint Base Langley-Eustis, VA. This acquisition is for 29 AMiON enterprise software licenses, including all-inclusive labor, equipment, tools, parts, materials, and preventative maintenance (PM) and corrective maintenance (CM) support necessary to ensure proper functionality of the AMiON enterprise software and/or hardware utilized by the 633rd Medical Group (MDG).� The vendor shall provide only AMiON enterprise Original Equipment Manufacturer (OEM) certified and qualified technicians authorized to perform services on the AMiON software and hardware.� The vendor shall provide all-inclusive labor, equipment, tools, parts, materials, and preventative maintenance (PM) and corrective maintenance (CM) support necessary to ensure proper functionality of the AMiON enterprise software and/or hardware.� Services will be performed at the 633rd MDG, Joint Base Langley-Eustis, VA.� Contractor shall be OEM authorized dealer, authorized distributor, or authorized reseller for the proposed software/hardware and equipment such that OEM warranty and services are provided and maintained by the OEM.� All software licensing, warranty, and services associated with the software/hardware and equipment shall be in accordance with OEM terms and conditions.
