Description
Page 5 of 5 Veterans Health Affairs, Network Contracting Office 5, is issuing this sources sought notice in order to identify sources that have the resources and capabilities to provide storage of healthcare furniture. STATEMENT OF WORK 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for healthcare furniture to be stored in warehouse facility provided by VHAWAS RADIATION ONCOLOGY Service. Healthcare furnishings include, filing and storage systems, modular cabinetry, modular systems, seating, tables/carts, furniture accessories and related services. The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and be responsible for the technical assistance, development and management of furniture inventory stored in warehouse facility. Period of performance: Base Year + 1 option year. Duty hours will be 7:30 am 4:00 pm. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Juneteenth, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide project management of all products and services to store, manage inventory, deliver from the manufacturer until full and final acceptance by DCVAMC Interior Design Department. Storage Provide adequate storage/square footage for 40 palettes of furniture Warehouse space/Square footage allocated for DCVAMC furniture should be clean, and free of dust and debris, and temperature controlled. All Furniture stored on behalf of DCVAMC should be stored together in one primary location unless communicated and approved by DCVAMC Interior Design point of contact. All bays where furniture store on behalf of DCVAMC should be clearly always labeled and accessible. Manage Inventory All DCVAMC furniture should be inspected for condition and quantity accounted for monthly. Furniture inventory lists to be send automatically to DCVAMC Interior Design Department on the 1st of each month the furniture is in storage. Report must include Item name Quantity Manufacture Dimension Part # Finish Condition Bay location Labor All persons assigned to move and deliver furniture stored by DC VAMC should be familiar with all furniture and warranties associated with the furniture. Technical leader/ foreman should be available upon request to provide feedback about parts and pieces, and repair of damages. Any damages incurred to DCVAMC Furniture and or property while in transit or delivery will be at the responsibility of Contractor Provide Delivery of Furniture to DCVAMC Furniture delivery will be completed in 3-6 scheduled deliveries All deliveries will be coordinated between DCVAMC Interior Design Point of Contact. Furniture should be requested DCVAMC and confirmed at minimum 5 days in advance via email or phone call to VAMC point of Contact. Furniture may be delivered to DC VAMC located at 50 Irving Street Washington, DC 20422 or another VA owned property or CBOC located with Washington, D.C., VA, OR MD. 4.0 General Requirements for Services: The dates and times for delivery, receipt and installation of furniture will be established by individual orders. Delivery, receiving and installation of furniture will be determined on the individual orders and may occur outside of normal duty hours to not interfere with patient care in accordance with the IDIQ order. a. When the Contractor requests to deliver, receive and/or install furniture outside normal duty hours. When these instances occur, a written request shall be submitted to the DC VAMC Interior Design Point Contact for authorization of the individual orders of this type. Written approval from the DC VAMC Interior Design Point Contact must be received before proceeding with the work. The request shall be submitted at least (5) five business days prior to the proposed performance of work, and it shall indicate the day(s), time, the amount of work, and the reason why performance is necessary outside normal duty hours. b. The Contractor shall be made aware of and must comply with local security policies and procedures in the task order. The Contractor will comply to these standards during all site visits on Governmental property. Contractor shall contact the DC VAMC Interior Design Point Contact at least 14 days prior to work start date to ensure compliance to any local security policies. c. The Contractor personnel shall present a professional appearance and shall have a uniform or some method of identifying them as an employee of the contractor. A VA badge will be provided and must be worn above the waist and be visible at all times while on government property. d. The Contractor shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate safety or environmental risks. e. The Contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, Building Codes, Life Safety Codes, Occupational Health and Safety Administration (OSHA), ADA/ADAG, permits, VA design standards (www.cfm.va.gov/til/dguide.asp), and instructions required for the performance of the duties in the Statement of Work (SOW). g. The Contractor is responsible for ensuring staff are trained and certified for planning and installing product to maintain the integrity of the manufacturer s warranty.
