- DESCRIPTION The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 6) has a need for an ongoing program to provide prescription eyeglasses and optician services to Veteran beneficiaries in seven (7) medical centers (Asheville VAMC, Durham VAMC, Fayetteville VAMC, Hampton VAMC, Richmond VAMC, Salem VAMC and Salisbury VAMC). In addition, services will also be needed at Community Based Outpatient Clinics (CBOC) and Health Care Centers (HCC) that provide optical services. The VA will provide existing space within each VAMC, CBOC or HCC in order to provide services to eligible Veterans entitled to government provided eyeglasses. Offerors must possess all licenses, permits, accreditations and certificates as required by law, shall meet all Federal, State, and local fire and life safety codes and shall perform the required work in accordance with the standards set forth in the current edition of American Standards Institute (ANSI) for Ophthalmic Lenses Recommendation: ANSI Z80.1 or latest edition and in accordance with Food and Drug Administration (FDA). The NAICS Code for this requirement is 339115 Ophthalmic Goods Manufacturing with a size standard of 1,000 employees. This acquisition is a 100% Service Disabled Veteran-Owned Small Business set aside. Under this solicitation the VA anticipates award of a single indefinite delivery indefinite quantity contract on a firm fixed price (FFP) basis. This contract will contain a five (5)- year base period, with one (1)- year ordering periods, and a five (5)- year option period, with one (1)- year ordering periods. The total duration of the contract will not exceed ten (10) years.
