AMENDMENT 0001:� The purpose of amendment 0001 is to incorporate the following: 1.� Extend the due date for quotations to DECEMBER 27, 2022 at 10:00 am HST. 2.� Revised paragraphs 1.1.4. and 1.1.9. of the Statement of Work. 3.� Responses to questions received by the contracting office, see below. Question #1:� Do you have a current incumbent? Response #1:� The current incumbent is Ortho-Clinical Diagnostics Singapore PTE Question #2:� If yes for Q1, what is the contract number? Response #2:� W81K0219D0003 Question #3:� If yes for Q1, what is the name of manufacturer and model No. of the existing Chemistry and Immunology Analyzers? Response #3:� Ortho-Clinical Diagnostics / Model: VITROS XT7600 Question #4:� Could you accept the two (2) Analyzers (one (1) Chemistry Analyzer and another one (1) Immunology Analyzer separately) for One (1) Chemistry and Immunology Analyzers? Response #4:� No.� The Government requires an integrated system. Question #5:� If I want to bid, should I submit a past performance statement, contractor and subcontractor certificates, a registration certificate, a price file, and a subcontract plan?� Or do you have any other necessary documents? Response #5:� Refer to the Addendum to 52.212-1 to see all the requirements. Question #6:� Is it only possible to visit the site after the contract?� What are the preparations or procedures if visiting the site is possible before the contract? Response #6:� A site visit will be held on 8 December 2022 at 1:00 pm KST.� Due to limited personnel at the lab to sign people in, we request only key personnel participate in the site visit.� The site visit will be limited to a total of three (3) people.� Request potential offerors drive their own vehicle and only one (1) per company.� Please RSVP to Ms. Chae Hui Kim at chaehui.kim.ln@health.mil no later than 6 December 2022 at 1:00 pm KST; provide the individuals name, company name, title, and contact information.� Ms. Kim will provide the individual with the specific documents they will need to bring with them and location of the gate. The revised solicitation (W81K0223Q0004_Amendment 0001withAttach1) has been uploaded to SAM.gov. ********************************************************* This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, if necessary, and award a Firm Fixed-Price contract for Reagents to include Chemistry and Immunology Analyzers for Brian D. Allgood Army Community Hospital, Korea. The solicitation number for this request for quotation (RFQ) is W81K0223Q0004. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06. The Standard Industrial Classification Code is 6515, size standard is 1,250 employees and the NAICS code of 325413. The provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial, and its addendum applies to this acquisition. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. FAR clauses that are applicable to this acquisition are in the attachment to the combined synopsis/solicitation. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a Firm Fixed-Price contract to the successful quoter.
