Description
STATEMENT WORK CONSTRUCTION INSTALL EMERGENCY DEPARTMENT SHORE POWER AT LAKE NONA PROJECT NO. 675-23-804 ORLANDO VA MEDICAL CENTER, 13800 VETERANS WAY, ORLANDO, FL 32827 DEFINITIONS AND ACRONYMS: Contracting Officer (CO): The services to be performed under this contract are subject to the general supervision, direction, control and approval of the Contracting Officer. Project Manager (PM)/Contracting Officers Representative (COR): The Contracting Officer's representative responsible for administering contracts under the immediate direction of the Contracting Officer. Contractor: This term, as used herein, refers to the contractor under this contract. Pre-Construction Risk Assessment (PRCA): Used to identify potential risks associated with the renovation, construction and some maintenance & repair activities and to develop risk mitigation strategies to minimize these risks. Prior to beginning work this assessment will be completed. Interim Life Safety Measures (ILSM): Measures that are instituted to compensate for significant hazards posed by existing life safety (fire) code deficiencies or construction/renovation activities. Request for Proposal (RFP): Is a solicitation made, often through a bidding process, by an agency or company interested in procurement of a commodity or service, to potential suppliers to submit proposals. National Fire Protection Association (NFPA): The world's leading advocate of fire prevention and an authoritative source on public safety, NFPA develops, publishes, and disseminates codes and standards intended to minimize the possibility and effects of fire and other risks. The Joint Commission (TJC): Formerly the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and previous to that the Joint Commission on Accreditation of Hospitals (JCAH). The Joint Commission provides evaluation, accreditation and disease-specific certification programs for healthcare facilities. Competent Person (CP): One who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. SCOPE OF WORK: General Requirements: The Orlando VA Healthcare System (OVAHCS) is seeking professional construction services, for the project entitled: Install Emergency Department Shore Power at Lake Nona located at the Orlando VA Medical Center, 13800 Veterans Way, Orlando, FL 32827. Contractor shall provide all construction services including labor, materials, tools, and equipment as necessary to construct the project described below and other specific tasks as further defined by this RFP. All proposals are to provide construction services for this project. Project Overview: This project will provide power for mobile intensive care units and other contingency equipment in the Emergency Department parking lot. A pad mount transformer and distribution panel shall be installed to meet this requirement. Specific Project Requirements: The contractor shall be responsible for establishing all electrical connections according NFPA 70, National Electric Code and existing industry standards. Contractor shall bore shot a circuit from manhole NA2-4 to the designated area in the Emergency Department parking lot. Contractor shall install and terminate a 15KVA circuit from CEP normal switchgear room 1A101 through undergrounds existing ducts to manhole NA2-4 and through the bore shot to the transformer location. The contractor shall install a service loop of cable in all manholes for this circuit. Contractor is responsible for providing and installing new conductors. The contractor shall size all circuit conductors in accordance with the National Electric Code (NEC). Prior to running any conductors through existing conduit, contractor shall field verify and inspect conditions of existing conduits. Contractor shall verify space capacity and verify existing conditions of conduit. In the event there is water or other obstructions (i.e. garbage), Contractor shall coordinate field-verification with Facilities Management and remove the water and/or obstructions prior to installing new conductors in the conduit. Contractor shall install a concrete pad with additional spare conduits, and install a 300 KVA pad mount transformer. The primary voltage for the transformer will be 12,470 volts, three phase delta and the secondary shall be 120/240 Volts single phase Delta. Contractor shall install surge protection on transformer (primary side). The contractor shall install a distribution panel for the transformer with a main circuit breaker for 1200 Amps. All work will be in accordance with the NEC and industry standards. Bid Deducts: Bid-Deducts shall be incorporated into this project for options of portions of the work to be executed as defined below. Contractor shall install alternate shorter circuit. Contractor shall begin circuit and terminate from Fisher House pad mounted transformer instead of beginning the circuit at the CEP normal switchgear room. Contractor shall use existing underground ducts and manholes to NA2-4 from Fisher House transformer location and bore shot a circuit from manhole NA2-4 to the designated area in the Emergency Department parking lot. RECORDS MANAGEMENT: Citations to pertinent laws, codes and regulations shall apply to this contract, such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. SCHEDULING, PHASING, AND WORKING HOURS: Design Phase: All design related work (e.g., site visit, meeting, etc.) at the VA facility MUST be coordinated with the project s COR. All work shall be performed during normal business hours (7:00 am to 5:00 pm, Monday through Friday). All requests for work after normal business hours, WHEN hours (Weekends, Holidays, Evenings and Nights) must be requested by the A/E contractor at 14-calendar days in advance. Construction Phase: All work hours MUST be coordinated with the project s COR. All work shall be performed during WHEN hours (Weekends, Holidays, Evenings and Nights) so as not to interfere with normal Hospital functions. However, some VA areas may be conducive to work being completed DURING normal business hours (7:00 am to 5:00 pm, Monday through Friday). All requests for work during normal business hours must be requested by the Contractor at least 14 calendar days in advance. Additionally, construction shall be performed (scheduled and phased) to ensure that exterior egress/ingress areas are completely operational by normal business hours the following workday. Length of Project Project will have a 150 calendar day construction phase. Please refer to Program Guide PG-18-15, Volume C for document requirements. Contractor shall schedule a Pre-Construction Meeting with all pertinent VA personnel within 14 calendar days of receiving Government Notice to Proceed. Contractor shall submit a Critical Path Method (CPM) schedule for BOTH Design phase and Construction phase with estimated dates and itemized cost of work to the Contracting Officer for review and approval before Notice to proceed. Utility Outages, including disruption of fire alarms, must be requested by the Contractor at least 14 calendar days in advance. PROJECT CLASSIFICATION: This project has been classified per the Construction Infection Control Risk Assessment as a Type C, Low Risk Group, Class I (ISLM Level I) Project. SAFETY REQUIREMENTS: All Contractor's personal are required to have completed an OSHA-approved 10-hour construction worker course prior to working on the site. The On-site general superintendent/supervisor is required to have completed an OSHA-approved 30-hour construction course prior to working on the site.  Every worker must provide the Contracting Officer a copy of current certificates identifying successful completion of the training prior to executing work for the Veterans Administration. In addition, Contractor shall provide submittals for contract construction or renovation work which include the names, qualifications, and training dates for Contractor s Competent Persons (CP), who has been designated to administer the site-specific safety program, as well as the CP for other activities as required by OSHA regulation 29 CFR 1926.32 (such as scaffolds, cranes, excavations, etc.).  Contractor s CP are required to implement and maintain effective safety programs that identify and control hazards that may cause injury or illness to VA patients, staff, visitors, and contractor employees.  The VA CP does not take the place of Contractor s CP nor acts on their behalf.  The VA CP determines if Contractor is meeting VA standards and contractual requirements for safety and OSHA compliance.  When these standards and contract requirements are not being met, the VA COR and/or CP will take immediate action to prevent injury, non-compliance, and/or property damage. The Contractor shall submit an Activity Hazard Analysis, and an Accident Prevention Plan as per FAR 52.236-13 prior to commencement of site activities.  The Government shall have 21 calendar days to review the AHA and APP.  GOVERNMENT FURNISHED INFORMATION: All available record drawings will be provided upon request to the Design-Build Team A/E. They should not however, be considered as-built . These drawing files are provided without warranty or obligation as to the accuracy or information contained in the files. All information in the files shall be independently verified by the user. Any user shall agree to indemnify and hold the VA harmless from any and all claims, damages, losses, and expenses, including, but not limited to, attorney fees arising out of the use of the Computer Aided Design and Drafting (CADD) drawing files.
