Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources-sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service-Disabled Veteran Owned Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) UEID number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. Please list work that your company has performed on contract within the last 5 years. Contractor shall perform onsite surgical instrumentation repair and preventative maintenance for the medical facility. This is a service contract. It will provide services two days a month. Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service-Disabled Veteran Owned Small Business set aside. However, if response by Service-Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 811210 ($30 M). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA). Additionally, all Service-Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit information by e-mail to Doreen.masterson@va.gov. All e-mails must have as the subject line 36C26223Q0330 Instrument Repair and Preventive Maint. (PVAHCS) . All information submissions must be received no later than 3:00 pm MST on January 3, 2023. Statement of Work General Objectives and Requirements Purpose: The Phoenix VA Health Care System (Agency) requires a contractor inspect and repair all surgical and dental instrumentation. Background. The Sterilization Processing System needs to continue the process of monthly inspections and repairs of all surgical and dental instrumentation due to mass passivation and replacement project we implemented in FY22. Scope. The contractor shall provide all labor, travel, equipment, and miscellaneous supplies to perform Instrument Inspection, Repair and Clean Services. Identify all non-repairable instrumentation. Identify and repair all necessary instrumentation and return to original equipment manufacturer specification (OEM). Ultrasonic clean to ensure no further residue remains before return to sterile processing department for reprocessing. Bi-weekly progress report through an Observations and Activity Report and submit to the Contracting Officer s Representative (COR). Restore instrument passivation. 12-month warranty against defeats. The contractor shall identify cracked, broken, and life worn instruments. Any instrument found to be unrepairable. The contractor site technicians shall maintain a dedicated inventory for Customer s Tray to ensure seamless tray restoration. The Contractor shall coordinate with the COR of his/her intent to come on-site before arrival. The Contractor shall check-in with the Sterile Processing Service upon arrival on station. The department is located in Building 1, Room 1403. The government reserves the right to increase or decrease the number of instruments under this contract. Security Requirements. Security Requirements for Unclassified Information: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. The vendor may not have physical administrative access to the equipment included in this contract that capture and process ePHI. Privacy and HIPAA Focused Training Web-Based (WBT): The official privacy training for VA contractors is VA Course No. VA10203. This training is not needed be completed when contractor needs access to medical facility for two or more consecutive days, or who will require access to VA information or systems in the performance of their official duties (e.g. CPRS or VistA Web). Course is available from the TMS website at:   https://www.tms.va.gov.  Certificate of training must be provided to COR upon request. The Contractor shall not remove any hard drives or other data storage devices off station. All data storage devices shall be turned in to Agency upon de-commission. Risk Control. The Contractor may need passes from the on-station Vendor Check in Station daily. Performance Monitoring. The Agency shall oversee work done by the vendor and accept the quality of work as appropriate. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). The Agency shall provide utilities necessary for contractor equipment. The Agency shall provide contractor physical access to the agency instruments. Other Pertinent Information or Special Considerations. Inspection and Acceptance Criteria: The Agency shall sign off on the work completed if service is determined to be satisfactory. Indian School Road Phoenix, AZ 85012 Government Holidays The Vendor is not required to provide service on the following National holidays, not shall the Vendor be paid for these holidays. The following national holidays observed by the Federal Government: New Year s Day 1 January Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Juneteenth 19 June Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day 4th Thursday in November Christmas Day 25 December If a holiday falls on Sunday, the following Monday shall be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Also included would be any day specifically declared by the President of the United States. VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
