Description
This Sources Sought Notice is issued in accordance with FAR 5.101.  The purpose of this notice is to identify potential sources providing Contract Community Nursing Home Services for the South Central VA Healthcare Network (VISN 16) throughout the catchments areas of the Gulf Coast Veterans Healthcare System, Biloxi, MS, the Alexandria VA Healthcare System, Pineville, LA, the Southeast Louisiana VA Healthcare System, New Orleans, LA, the Overton Brooks Veterans Healthcare System, Shreveport, LA and the Veterans Healthcare System of the Ozarks, Fayetteville, AR throughout the duration of the period of performance.  The South-Central VA Health Care Network (VISN 16) is one of 18 Veterans Integrated Service Networks (VISNs) of the Department of Veterans Affairs (VA). VISN 16 serves Veterans in Arkansas, Louisiana, Mississippi, and parts of Texas, Missouri, Alabama, Oklahoma, and Florida. VISN 16 serves a population of approximately 1.4M Veterans and more than 450,000 Veterans received care at the eight VAMCs and 61 outpatient sites of care in VISN 16, annually. Interested agencies will be Medicaid approved as applicable to the location as a Certified Nursing Home Facility and licensed as a Nursing Home by the State in which its provider number is based. The estimated period of performance will be from the Award Date for a period of 5 years. The number of veterans requiring Community Nursing Home services (bed days) over the next five years will fluctuate and cannot be adequately determined at this time. Therefore, the award of a Fixed Unit Price Indefinite Delivery Contract (IDC) is anticipated. Please be aware all Federal, State and local labor laws will be applicable to this procurement including federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act. If you have specific questions regarding labor laws, you should contact your local Federal Department of Labor representative. VA will utilize the appropriate Resource Utilization Grouping (RUG) rate(s) as a guide to determine VA rates. The VA rate will include the cost of medical care, routine medications, laboratory, x-ray, therapy (ies), and other special services authorized by VA, unless otherwise specifically exempted. VA will contract for appropriate categories of care using Resource Utilization Groups (RUGs-IV) as a reference or guide. This Sources Sought notice is a means of conducting market research to identify capable, experienced parties able to effectively and efficiently perform the objectives listed herein.  The result of this market research will contribute to determining the method of procurement and acquisition strategy.  The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 623110.  This notice does not commit the Government to issue a solicitation or make an award OR to prelude a solicitation expected to be issued from closure of notice. Description of Services: Community Nursing Home is a place where Veterans can live full time and receive skilled nursing care any time of day or night. VA contracts with community nursing homes to care for Veterans. The Community Nursing Home program is offered in many communities so Veterans can receive care near their homes and families. Community Nursing Home care always provides: 24-hour skilled nursing care (such as wound care or help with IV medication) Occupational and physical therapy Access to social work services Some Community Nursing Homes also provide: Short term rehab services Hospice and palliative care for the end of life Special care for dementia Refer to the attached draft Performance Work Statement (PWS) for more detail and required qualifications.  The Government intends to award multiple Fixed Unit Price Indefinite Quantity (IDC) with Economic Price Adjustment (FP-EPA IDC) Type contracts from any resultant solicitation. Pineville, LA, serving the State of Texas counties of Sabine, Desoto and Natchitoches and the State of Louisiana parishes of Vernon, Beauregard, Allen, Evangeline, Jefferson Davis, Calcasieu, Winn, Grant, LaSalle, Catahoula, Avoyelles, Rapides, St. Landry, Acadia, Lafayette and St. Martin. Southeast Louisiana VA Healthcare System, 2400 Canal Street New Orleans, LA, serving the State of Louisiana parishes of St. Tammany, East Baton Rouge, West Baton Rouge, Lafourche, Terrebonne, St. Charles, Iberville, Livingston, Tangipaho, Ascension, Assumption, St. John the Baptist, La Place and St. Bernard; Overton Brooks VA Medical Center 510 E. Stoner Ave, Shreveport, LA serving the State of Louisiana parishes of Caldwell, Franklin, Madison, Jackson, Ouachita, Union, Moorehouse, West Carroll, East Carrol, Richland, Tensas to Caddo, Bossier, Webster, Claiborne, Lincoln, Bienville, and the State of Texas counties of Red River, Sabine, Desoto and Natchitoches and Veterans Health Care System of the Ozarks, 1100 N. College Ave., Fayetteville, AR serving the surrounding areas of Branson, Joplin and Springfield, MO, Fort Smith, Harrison, Ozark and Fayetteville, AR and the area surrounding the City of Jay, OK. Instructions to Industry: All capable parties are encouraged to respond. Responses must directly demonstrate the company s capability, experience, and ability to effectively and efficiently perform the objectives contained herein. The Government requests interested parties submit a written response to this notice which includes: Community Nursing Home (CNH) name, address, email address, website address, telephone number and type of ownership for the organization of the facility where services will be provided. BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether the CNH is a large or small business as defined by the NAICS (b) If small, indicate if your CNH qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your CNH is certified under Section 8(a) of the Small Business Act (d) Indicate if your CNH is a certified Hub-zone firm (e) Indicate if your CNH is a woman-owned or operated business (f) Indicate if your CNH is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) Note: As first announced on September 23, 2022, VA has ceased accepting new applications for Veteran-owned small business verification or reverification as of October 24, 2022. This action was taken to facilitate the January 1, 2023, transfer of CVE s verification functions to the Small Business Administration (SBA) as mandated by the 2021 National Defense Authorization Act. New applications should be submitted to SBA when they begin receiving new applications on or around January 2023. To be eligible for award, contractors must be registered in the System for Award Management database. Registration is free and potential offerors are encouraged to visit the SAM website as no IDCs will be awarded to any offeror not registered and current in SAM. Attestation that the facility has received certification from Centers for Medicare/Medicaid Services (CMS) under Title XVIII (Medicare) or Title XIX (Medicaid) (as applicable) or have received special approval from the Office of Geriatrics and Extended Care (GEC) Office Copy of current and unrestricted state license to operate as a skilled nursing facility *Responses must be submitted via email to the Primary POC no later than the specified closing date of this notice.  **NO QUESTIONS WILL BE ACCEPTED. The Government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice and otherwise anticipated; Disclaimer and Important Notes:  This notice does not obligate the Government to solicit or award a contract. Any information provided by industry to the Government as a result of this sources sought synopsis is strictly voluntary. Responses will not be returned. No entitlements to payment of direct or indirect costs or charges to the Government will arise as a result of contractor submission of responses, or the Government's use of such information or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization s qualifications to perform the work.
