- This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is 36C24523Q0161 and is issued as a Request for Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2021-07, fiscal year 2023. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. Background The cardiac catheterization lab at the Baltimore VA Medical Center has recently gone under renovation construction. A Stryker S-series boom and a Stryker single common plate were provided and installed by the general contractor to support the future installation of two 32 4k surgical monitors on suspension arms and a set of boom-mounted drawers. Scope All equipment must be new from the original equipment manufacturer or an authorized distributor The contractor shall provide equipment, shipping, and installation services necessary to meet the requirements and capabilities listed within the scope The assembly and validation of the system must be performed by a vendor service trained representative. Equipment Characteristics: One (1) monitor mounted to Stryker single common plate: One (1) 32 4K surgical display with two (2) HDMI and one (1) DVI video inputs One (1) display mount and suspension arm mounted to a Stryker single common plate and capable of 540° of rotation One (1) handle Associated display cables from video source, through interstitials and suspension arm, to monitor One (1) monitor TC mounted to Stryker S-Series boom One (1) 32 4K surgical display with two (2) HDMI and one (1) DVI video inputs One (1) Suspension arm mounted to Stryker S-Series boom TC base and capable of 540° of rotation One (1) handle Associated display cables from video source, through interstitials and suspension arm, to monitor Set of drawers mounted to Stryker S-Series boom One (1) drawer frame mounted to Stryker S-series boom One (1) 6 depth drawer (no lock) One (1) 9 depth drawer (no lock) Delivery and Installation The contractor shall coordinate equipment delivery and installation with the Biomedical Engineering team at the Baltimore VA Medical Center or the COR of this procurement to minimize impact to patient care The contractor shall ship delivery materials in manufacturer s original sealed containers with the associated purchase order number listed on the outside of the box The contractor shall deliver all equipment and items required for installation to the following address: Baltimore VA Medical Center 10 N. Greene Street Baltimore, MD 21201 Security Requirements The contractor must have a photo ID available when performing work at the VA Medical Center and be willing to present it to VA staff. The contractor is responsible for providing any personal protective equipment (PPE) that is deemed necessary to complete tasks stated in this requirement. The contractor must follow the VA Maryland Health Care Systems current COVID-19 guidance for entry to any medical center or Community Based Outpatient Clinic (CBOC). Question 4: For each of the items below, what is the source of the Product Number? The referenced part numbers are Stryker part numbers Question 5: Who is the manufacturer of this item? (FS 1203 seems to be a local stock number that only has meaning for a specific vendor) This is a Stryker part number Question 8: What are the specific multiple input options? 2 HDMI and 1DVI per monitor Question 9: What is the 1688 4K system? It is not otherwise described in this solicitation. 1688 4K system is an endoscopy platform sold by Stryker. The platform itself is not part of this requirement but the monitor should be compatible with that platform Question 10: What is meant by 9 preset surgical specialties?"" Where are these 9 presets defined? This refers to 9 preset display configurations Question 11: What is the make and model that this mount is for? This is to mount to a Stryker S-series boom Question 12: What is the make and model of the suspension cable kit? This is a Stryker cabling kit (Part number CY 9000102) Question 13: Is there a description of its functional characteristics? These are display cables that are run through the suspension arm that the surgical monitor is mounted from Question 14: What is the make and model, or description, of this kit? This is a Stryker cabling kit (Part number P42604) Question 15: What is the length of this cable? 45 Question16: What is the meaning of DVI Upgrade Extra Monitor? These are DVI extenders/encoders to display the image on additional monitors Question 17: Does this apply to the 32 monitor in item 1? This requirement allows us to extend the DVI signal to secondary monitors like the one that is part of the cath lab equipment Question 18: What is the cabinet or unit that this drawer is to fit into? This is to mount to a Stryker S-series boom (using the C-frame referenced above) Question 19: What is the cabinet or unit that this drawer is to fit into? This is to mount to a Stryker S-series boom (using the C-frame referenced above) Question 20: What is the definition of PKG or Package in this description? The Package should include the 32 surgical monitor and the monitor bracket to mount to the suspension arm Question 21: Is that an add-on option to the 32 display described above?
