- This Sources Sought/Request For Information (RFI) IAW FAR 13.104 Promoting Competition is not a request for quotes, but rather a Request For Information (RFI)/Sources Sought Notice. It is at the Government s discretion to consider responses to this notice. This is an Unrestricted/Open Market Sources Sought/RFI for Brand Name Only services. Only Brand Name services and no other services will satisfy agency requirements. Boise VAMC intends to procure a Base Year Plus Four (4) Option Years service contract to furnish: Shimadzu SonialVision G4 Site ID: 778 S/N M1C356487001 & Associated Canon DR system Site ID: 778 S/N 112102. The projected Base Year delivery date will be 3/29/2024 and Period of Performance of 3/30/2023-3/29/2024. This purchase will be made utilizing Simplified Acquisition Procedures in accordance with Federal Regulation (FAR) Part 13 and/or FAR Part 16 as applicable. Interested parties may identify their interests and capabilities by responding to this requirement by the due date listed on this notice. Responding vendors must include a current dated Letter of Authorization to perform services required for Shimadzu SonialVision G4 Site ID: 778 S/N M1C356487001 & Associated Canon DR system Site ID: 778 S/N 112102 from OEM Shimadzu Precision Instruments. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Information received will be considered solely for the purpose of determining whether or not to conduct a competitive procurement. A determination by the Government not to compete this proposed award upon response to this notice is solely within the discretion of the Government. No reimbursement for any cost connected with providing capability information will be provided. Contract Title. Maintenance and Repair Service Contract for Shimadzu SonialVision G4 Site ID: 778 S/N M1C356487001 & Associated Canon DR system Site ID: 778 S/N 112102 for the Boise VA Medical Center (BVAMC). Base year plus 4 Option Years. 3. Background. Biomedical Department does not have the necessary specialized tools and training to perform all maintenance and repair of the Shimadzu Sonialvision G4 system. Qualified Service is required to maintain the system to Manufacturer s specifications. The equipment is located in the BVAMC s Radiology Service. Sensitive Patient Records may be viewed as a result of this requirement. Access to the controlled area will be provided by POC, Radiology Staff, Police Service, or another designated and authorized BVAMC employee. 4. Scope. Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies to furnish preventative maintenance services, and repair services for the Siemens Artis Zee system with functional location 400-572434. All equipment is located at the BVAMC Radiology Service Department, Building 85. Emergency Repair service will be provided from 08:00 AM to 05:00 PM Monday through Friday. 5. Specific Tasks. The service covered under the contract includes the following: Coverage includes (during Standard Coverage Hours): . Emergency repair labor . Travel to and from customer site . Equipment inspection . Equipment recalibration . Operating System updates, as prescribed by imaging software manufacturer . DR software updates . Preventative maintenance inspections per manufactures recommendations . Hard Drive cloning performed during Preventative Maintenance . Loaner detectors (for a discounted fee), if available . Loaner Portables (for a discounted fee), if available . All Parts Including Glassware Exclusions: . Consumable items plastic accessories, batteries and other consumable items. . Work and travel outside of Standard Coverage Hours . Drop Coverage protection 6. Performance Monitoring Performance will be monitored by periodic inspections and observation of work. Work will be observed by the POC, or designee, during performance of scheduled maintenance service and inspections and/or during repair service. Equipment operators will be contacted to report on contractor performance when POC, or designee, is not available. 7. Security Requirements. See contract VA797H-17-D-0012 for required security language. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). No Government supplied Equipment or Information is required or anticipated. 9. Other Pertinent Information or Special Considerations. Only Manufacturer Trained Service Engineers/Technicians or Manufacturer Authorized Service Engineers/Technicians will perform services and repairs. a. Identification of Possible Follow-on Work. No Follow-on work is anticipated. b. Identification of Potential Conflicts of Interest (COI). No Known COI c. Identification of Non-Disclosure Requirements. Contractor will have access to Sensitive or Proprietary Information. Any Patient Information that may have been observed during the course of the work must not be discussed, or shared, with anyone beyond the Hepatology Service Staff or any person who does not have the need to know such information. d. Packaging, Packing and Shipping Instructions. N/A e. Inspection and Acceptance Criteria. POC, or designee, will verify that the unit is working within Manufacture s specifications after completion of service or repair. 10. Risk Control. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor s fault or negligence. In the event that Patient Information may have been observed during the performance of work, the Contractor will not discuss any patient issues that may have been observed during the course of the work. 11. Place of Performance. All service will be performed at the BVAMC Radiology Service. 12. Period of Performance. Base Year: 3/30/2023 - 3/29/2024 Option Year 1: 3/30/2024 - 3/29/2025 Option Year 2: 3/30/2025 - 3/29/2026 Option Year 3: 3/30/2026 - 3/29/2027 Option Year 4: 3/30/2027 - 3/29/2028 13. Delivery Schedule. Service Reports will be emailed or faxed to POC by the 5th workday after completion of the work.
