UPDATE 10 JAN 2023 The purpose of Update 10 Jan 2023 is to extend the due date to Monday, 30 Jan 2023, 12:00 pm MST.� To submit response to solicitation, please email the completed Attachment 2 Catholic Priest Certification of Qualifications to both the contract specialist Rony Exantus at email rony.exantus@us.af.mil and contracting officer Dawn Schmidt at email dawn.schmidt.1@us.af.mil.� Thank you. ORIGINAL NOTICE The 75th Air Base Wing Chaplain Program, Hill AFB UT, intends to establish multiple award Blanket Purchase Agreements (BPAs) to provide Auxiliary Catholic Priest Services during the absence of the active duty Catholic Priest. The Multiple Award BPAs for Hill Air Force Base (AFB), Utah (UT) will be for a 2 year ordering period, 15 Jan 2023 to 14 Jan 2025. The contractor shall provide all personnel, labor, equipment, tools, materials, supervision and other items required to supply and maintain requirements defined in the Performance Work Statement (PWS) dated 15 Nov 2022. The following attachments are provided to prepare quote: 1. PWS 15 Nov 2022 2. Catholic Priest Certification of Qualifications 3. Aux Catholic Priest BPA Terms and Conditions and Ordering Procedures 4. PWS Safety Appendix 5.
