Description
Mesa Recovery Center Modifications Project 575-23-103 OBJECTIVE: Provide labor, materials, equipment, and other services as necessary for minor modifications of the West and South wings of the 5th floor in Building 1 at the Grand Junction VA Medical Center campus, 2121 North Ave., Grand Junction, CO 81501. SCOPE OF WORK: Work includes installation of infection control mitigation measures; selective demolition; plumbing repairs, bathroom renovations, wall tile repairs, floor repairs, new door hardware, an additional security camera and new plumbing fixtures. � � �� REQUIREMENTS/MANDATORY TASKS: � � The contractor shall complete all requirements and mandatory tasks listed below. Furthermore, the contractor shall provide all necessary services, materials, tools, equipment, bonding, and insurance for this project. Drawing Reference # Description of Requirements and Mandatory tasks Locations/Rooms 1 Replace door slab to add mortise keyed locks to patient rooms. Manufacturer to be ""Best SPSL secure patient safety lever"", BSS finish with 12"" kickplate BSS.� Phasing info: only one room may be taken out of service at a time to allow this work to be completed. Five business days �notice required before taking the room out of service. Patient rooms 5001, 5002, 5403, 5411, 5412, 5413; 5419. 2 Redo shower walls and epoxy floor.� Room 5419a 3 Repair walls tile holes in restrooms Phasing info: only one group restroom may be taken out of service at a time to allow this work to be completed. �Five business days-notice required before taking the room out of service. Rms; 5000, 5001, 5002, 5003, 5403, 5414, 5419a, 5421, 5426, 5431a, 5431x. 4 Install 4'x8' white board over medical gas headwall in therapy rooms. Phasing info: only one therapy room at a time can be taken out of service to install cover. �Five business days-notice required before taking room out of service. Rooms 5201, 5202, 5203, 5204, 5217, and 5206 5 Replace horizontal window blinds with vertical no cord blinds. Rooms 5201, 5204, 5209, 5417, 5418 (current blinds split), 5428 6 New wall sinks restrooms 5424 & 5431x Phasing info: only one group restroom may be taken out of service at a time to allow this work to be completed. �Five business days-notice required before taking the room out of service. Restrooms 5426, 5431x 7 Shut down 5th floor water main and repair room 5419a hot water recirculation line, by adding (3) new plumbing isolation line valves (domestic cold, domestic hot, domestic hot re-cric.) & valve(s) access panel. Rooms 5413, 5424 & 5431 replace Sharkbite fittings with soldered or pro-press fittings. Phasing info: one room at a time can be taken out of service in order to rough in plumbing, using room isolation valve, with five business days-notice for each individual room. �21 days-notice is required for the 5th floor water shutdown to repair hot water recirculation system. Rooms 5419a, 5431, 5431x, 5413, 5424, 5426, 5428 8 Add plexiglass with voice hole and paper pass thru at nurse stations 5005 (9'x4') & 5420 (6'x4'). Rooms 5005, 5420 9 Add 360 deg PTZ fisheye camera in south corridor outside room 5201 Corridor outside room 5201 10 Construction barriers and infection control measures As required project wide 11 Install sink on east wall. �Remove panel and turn over to facility. Need sink type and check specifications. 5420 12 Remove and replace bathroom sink/mirror with a new � residential style sink/mirror. Look at specifications �5212, 5214, 5210, 5403 13 Match white finish/style/type of led wall mounted ceiling lights. Replace existing wood plastic trim fixtures with white finish plastic led wall mounted lights.� 5411; 5412 14 Remove small wall mounted light. Patch and paint wall. 5431 15 Demo corridor water fountain. 5430 16 Remove wall mounted stainless steel speaker. �Patch and paint wall. 5424 17 Demo sink. Cut lines into wall. �Cap lines in wall. �Disconnect from main water line. �Cap lines at disconnect. �All caps will be black cast iron plugs. Check specifications with plumbers. Place panel over wall location. Match panel in room 5417. Outside room 5217, and inside rooms 5201, 5204 18 Install water dispensing station. � Corridor outside 5204 19 Expand HDMI ports via HDMI splitters (1080p 1x4 4 port HDMI extender splitter HDMI signal distribution amplifier over cat6e/cat7 ethernet cable IR remote control (1 in 4 out) �up to 40m (130ft) at 1080p 60hz (hdmiext4p or equal) on CCTV receiver in room 5005. Install (2) 36 �led monitors with wall mounts included and connect the CCTV to 5204 & 5206. Requires (2) 100 ft of HDMI cable. Install an extension cord in 5204 & 5206. 5204, 5206 SPECIAL CONDITIONS: The area is a residential care area. �The contractor will need to coordinate hours and days for work with the VA engineering department, VA safety department and the VA hospital staff, especially work that exceeds 120 dB. � A pre-demolition study for hazardous materials has been performed on this building. �No asbestos nor lead paint is known to be in the area of renovations. �If asbestos is discovered in currently covered areas (pipe insulation, etc.) it will be remediated by the contractor under modification to the contract. All contractor and sub-contractor workers must attend a station orientation training class before they do any work on site. �Training takes approximately 2 hours and must be requested through the Contracting Officer s Representative (COR). �See specification section 01 35 26 part 1.8 paragraph A. Infection Control is very important. �Mitigation measures are required. �See specification section 01 35 26 part 1.12. Laydown and storage for construction is the responsibility of the contractor and must be off station. Payment can be made for off-site stored materials � see specification section 01 00 00, General Conditions part 1.6. Demolished and waste material must be taken out of the building via covered carts after 5:00 PM. �Transport of new materials and waste is covered under Infection Control. All inspections (fire sprinkler system, plumbing, electrical, HVAC, framing, doors and relites, communication system, access control) shall be done by the VA. �Notification for inspections must be given to the COR in writing at least 5 business days prior to the proposed inspection date.� The video surveillance modifications must be made so that there is no direct view of patient treatment areas, patient sleeping quarters, or toilet/shower rooms. No tobacco or other substances legal in the State of Colorado may be consumed by any method while on Station, including inside vehicles or construction trailers. �No smoking; no vaping; no chew. GENERAL: The Dept. of Veterans Affairs (VA) requires strict adherence to the Code of Federal Regulations 29, Part 1926. �This includes the requirement that the contractor have a �Competent Person �on the job at all times (must have a 30-hour OSHA card), and that all contractor employees have received a minimum of ten (10) hours of OSHA construction safety training. � This entire project shall conform to technical specifications, Federal Regulations, OSHA, AIA Construction Guidelines, Joint Commission Environment of Care Standards, and applicable building codes. �In addition, any part of any VA specification that is not up to current design and construction practice shall be brought to the attention of the Contracting Officer (CO) and COR for further evaluation. � PROJECT MANAGEMENT: Based on the above list of work items the general contractor selected must successfully manage a variety of trades and tasks, coordinate just-in-time delivery of materials, get those materials to the job site, and schedule the necessary labor to construct various parts of this project in a timely fashion. �Therefore, a critical aspect of this contract is construction management including but not necessarily limited to: Scheduling using the critical-path-method. Running and documenting an OSHA-compliant construction safety program. Managing job site access and security. Compliance with all VA security policy and Police requirements, to include badging of all personnel working on site. Managing the separation of construction activities from on-going hospital patient-care operations. Managing periodic utility shutdowns including appropriate permitting procedures including lockout/tag out, hot work, etc. Implementing and maintaining required interim life safety measures (ILSM) as needed Coordinating with VA COR to maintain acceptable conditions for all on-going hospital functions. Coordinate all training with COR Attend weekly construction meetings on phone or in person SUBMITTALS: Contractor shall be expected to submit the following: Log of all anticipated submittals Request for information (RFI) log with all responses Schedule of all work to include sequencing durations, critical path, and cost loaded schedule of values. With every invoice a new construction schedule will be submitted. Environmental compliance and monitoring plans� Interim Life Safety Measures (ILSM) for all related construction procedures 2 to 4 week look-a heads of anticipated work throughout the duration of the project List of all employees to be working on site and proof of their OSHA certifications (10 or 30 hour) Name of site superintendent and proof of 30-hour OSHA certification. (i.e., please be advised the site superintendent is required to be onsite at all times). HOURS OF WORK:� Work will take place during normal business hours (7:00 AM to 5:00 PM), Monday through Friday, all Federal Holidays excluded. �Evening (between 6:00 PM and 9:00 PM) or weekend work is discouraged, to avoid disturbing residents, but is allowed upon approval from the COR. Written notification must be given to the COR at least ten working days before the time requested.� ACCESS: � The COR shall provide the Contractor access to all buildings and rooms required in the provisions of this contract. The Contractor shall notify the COR when leaving these areas. The Contractor and all personnel shall comply with the security clearance or access controls of areas within the Medical Center. If the Contractor must obtain access to areas that require special security precautions, the COR will arrange for access PERIOD OF PERFORMANCE: Period of performance including lead time for materials and submittals: 180 days from Notice to Proceed. QUALITY CONTROL: The Contractor shall have a quality control program to assure the requirements of this contract are provided as specified. The Contractor and COR will inspect the work. Deficiencies will be noted and corrected before beginning work in another area. Any damage done by Contractor will also be noted and corrected by the Contractor within 2 working days. Damage causing an immediate impact on the environment of the hospital shall be corrected immediately. ACCEPTANCE STANDARDS: The VA reserves the right to dispute completion of contract scope if contract work is found not in accordance with manufacturer recommendations, or is not satisfactorily executed in accordance with the SOW. All work completed by the Contractor must be reviewed and approved by the COR prior to final acceptance.
